      • A few of the bleach bottles do not spray or when sprayed no bleach is detected. The concentration of the bleach was tested with the lid twisted off and the concentration for all bottles was at 100 ppm.(Kinder room, Toddler Room, Preschool Room, and Flex Room).**Clean the bottles or replace all the bottles. Ensure bottles can spray and that the straws are clean so bleach is detected with sprayed.**

      • **Repeat violation observed Feb 4th 25**The high temperature commercial dishwasher for sanitizing toys was not operational. on discussion with operations manager, it was noted that parts have been order with a 10 day ETA. Repair the dishwashers as soon as parts arrive. Continue with manual toy washing and disinfection in the interim.

      • Sink in the Kinders room is not operational.Ensure that all soap dispensers are cleaned as there was build up evident on the pump dispensing area.Ensure that supplies are not stored around the sink.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Sanitizer is not operational; Repair or Replace the sanitizer. Ensure that manual toy washing is conducted in the interim as discussed.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • A temporary awkwardly placed partition has been placed in an open bathroom area in the kinders room to provide privacy. Consider installing a permanent proper bathroom divider for children to safely access toilets and provides sufficient privacy as discussed.

    • 26. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Critical)?
      • Evidence of mouse infestation found in kitchen area (under the sink) and a dead mouse was found in the laundry room. No evidence found in classrooms.1. There is a pest control program in place. Please ensure that company reviews and addresses any points of entry and seals them. 2. Clean up mouse droppings using a soaked paper towel, and use 1000 ppm bleach solution to disinfect the areas. Discard dead mouse. 3. Ensure that all items stored under sinks and storage room are stored within clear totes with lids. 4. Monitor activity by assessing for new droppings after they have been cleaned.
