      • Senior B Room: broken mouse was used as a play equipment. Mouse cover was missing and had sharp edges. Mouse was discarded during inspection. Ensure broken equipment or toys are discarded and not used for play.

      • Senior A Room: Storage cabinet is not locked, and staff use it to store personal belongings (purse/handbag etc.). Ensure storage cabinet is kept locked at all times to restrict client access to potential hazards.

      • Senior A Room: Laundry bin is damaged. Ensure laundry bin is replaced.

      • The hand washing sink in the washroom & Junior room of this social care facility did not have adequate supplies.Ensure every hand washing sink is adequately supplied with liquid soap & single use paper towels in a suitable dispenser.

      • Dirty/soiled laundry bins in all the rooms are not labeled. Ensure dirty/soiled laundry bins are labeled.

      • Toddler Room A: Playmats are torn, and foam is exposed.Ensure playmats are replaced.

      • Toddler Room A: Cardboard is used as a barrier to prevent clients from accessing electrical outlets underneath the table. Ensure cardboard barrier is removed as it cannot be cleaned or sanitized. Replace with it with a suitable material that is smooth, impervious to moisture and easily cleanable.

      • Hand washing sink: Washroom & Junior Room: liquid soap dispenser was empty. Staff are not using soap to wash hands. Extra liquid soap refill was not available on site, so staff had to purchase some liquid soap during inspection. Ensure hand washing sinks are always adequately supplied with liquid soap and single use paper towels to facilitate proper hand washing. Extra supplies must be available on site.

      • Sanitizer spray bottles in client rooms are not labeled or incorrectly labeled & contains straight bleach without any dilution. Ensure bleach solution is 100 ppm and spray bottles are labeled as sanitizer (either chlorine sanitizer or hydrogen peroxide sanitizer). Staff must be adequately trained on preparing a sanitizer solution.

      • Junior Room: play equipment (mirror) is not stable and is a fall hazard. Ensure mirror anchored to the wall.

      • In Toddler rooms AA few of the sleeping mats are repaired with duct tape, making the mats difficult to clean. Please remove the duct tape and repair or replace the mats to be smooth, durable, impervious to moisture and easily cleanable.
