    • 06. Are proper protocols being followed in regards to client care?
      • The diaper change pad in the baby room is not attached to the surface, the belt that is attached to the surface was tucked under. Spoke to supervisor that if a child is being diapered up top on the change table vs the floor this attached belt must be out and ready for usage to go up and over the child to secure them in place The diaper change pad in the preschool area large, shared washroom was loose on the surface. The belt that is attached to the diaper table was in locked position. Same thing applies, the belt must be out and ready for usage to go up and over the child to secure them in place.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • ** Area rugs throughout need more attention to deep cleaning* The green cushion couch was visibly dirty in OSC: clean or discard * The door in toddler room that separates from preschool room: had a deteriorated surface. Replace or repair so smooth durable and easy to clean.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • Out of school care bathroom:Chemicals including toilet bowl cleaner were observed under the hand sink and accessible to children.Ensure all chemicals are kept out of reach of children at all times.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • NEW: Light blue couch in preschool room was observed to be stained. Discussed cleaning - laundry is indicated to be completed but stains and dirt observed.OUTSTANDING - Preschool washroom: back splash behind the faucet was in disrepair, caulking torn drywall was damaged and paint chipped. Repair surfaces, install a backsplash and finish the area so that the sink area is smooth, durable and easy to clean. July 2024: Above concerns remain outstanding. Unkept rugs throughout, stained, sticky stains, debris beneath all. Reduce the number of plush items, rugs. Replace with smaller rugs that can be tossed into washing machine.OUTSTANDING: Carpets in rooms noted to be stained. Large number of stuffed animals observed in infant room and preschool room.
    • 23. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Critical)?
      • Infant room: White crib - the mattress had gaps greater than 2 inches on the sides which can cause a hazard.The crib mattress was ripped in corner. Please replace the crib mattress with a proper fitting crib mattress that has a cover that is washable and water resistant.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Infant room and toddler room: Diapering pad was too large for the diapering surface which could lead to a fall hazard or injury.Please ensure that the diapering pad is safe and able to be restrained on the diapering surface.

    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • OUTSTANDING - Preschool washroom: back splash behind the faucet was in disrepair, caulking torn drywall was damaged and paint chipped. Repair surfaces, install a backsplash and finish the area so that the sink area is smooth, durable and easy to clean. CORRECTED Visibly dirty surfaces such as walls, doors, baseboards throughout the facility. Damaged walls covered in foam play tiles. Damaged & dirty baseboards. Surfaces including baseboards must be cleaned with warm soapy water on a regular basis. Repair and or replace finishes such as baseboards, walls, doors so that they are smooth and easy to clean. OUTSTANDING - OSC couch: An abundance of crumbs, dirt, food stains beneath cushions. Remove the couch or put a cover on top so that it is easier to keep clean. Unkept rugs throughout, stained, sticky stains, debris beneath all. Reduce the number of plush items, rugs. Replace with smaller rugs that can be tossed into washing machine.

    • 06. Are proper protocols being followed in regards to client care?
      • Baby room: there was a spray bottle, no label and the spray nozzle was not working. As per staff this was the dedicated bleach spray for the diapering surface. Staff brought over another spray bottle from toddler room: no label, tested and 0% bleach and there was no suction straw within therefore not functional. Staff found a new spray bottle, filled with bleach mix and labelled. Diaper surface was immediate disinfected. The diaper pad in baby room: wall mount change table with attached belt had an unattached belted diaper pad ontop leaving the diaper pad/child not secured to the surface. In order for a child to be secure the diaper pad needs attaching to the table surface or the belt that is attached to the table needs to be pulled up and used over the diaper pad. Or use the diaper change table as is without the extra change pad. Large preschool area washroom: There was no bleach solution adjacent to the diapering area. Staff found a spare empty spray bottle, labeled and filled with bleach mix for the diapering area. Toddlers are diapered in this washroom. The diaper pad in this washroom had a baby bottle laying on it. Keep non diaper items away from the diaper change table
