    • 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
      • Facility did not submit the personal services notification. The notification was submitted during inspection onsite.Ensure a PSN is submitted for each new service added to the facility.
    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility specific written procedures were not completed. AHS templates for specific procedures of skin cleansing, client care in case of accidental cut and decontamination of dual purpose sink were provided, reviewed and completed onsite during inspection.Ensure the procedures are being followed and available in the facility for review for new hired staff as well as for PHI for next inspection.
    • 20. Is reusable equipment disinfected properly?
      • 1- No disinfectant was noted in the facility to disinfect the clippers. Operator obtained a intermediate level disinfectant Clippercide with 10 min contact time during inspection onsite. The MIFU of the disinfectant were reviewed during inspection with operators.Ensure all clippers are properly cleaned and disinfected between clients.2- A disinfectant without drug identification number was noted onsite. The disinfectant selection process was reviewed with operators. The disinfectant was removed from the facility.Obtain all disinfectant with DIN and use them according to their manufacturer instructions for use.
    • 20. Is reusable equipment disinfected properly?
      • 1- Manufacturer instruction for use (MIFU) were not being followed for preparation of Barbicide disinfectant solution. MIFU have been reviewed from barbicide bottle label to prepare solution accordingly. 2-Barbicide solution jars were half to 3 quater filled which were not enough to fully immerse the combs. Parts of combs were noteed being out of disinfectant solution. Ensure combs are fully immersed in the disinfectant for required contact time.3- No disinfectant was noted onsite for disinfecting the environmental surfaces such as chairs, tables, counter spaces. Obtain a low level disinfectant or wipes to disinfect the surfaces between clients. "How to select right disinfectant" resource was reviewed with staff and copy left onsite for future reference.4-Hair accessories (rollers) were observed being manually cleaned and stocked in the container for reuse without being washed using soap and water and disinfecting. Ensure all hair accessories and combs are being properly washed with soap and warm water and disinfected in between clients.5-A disinfectant bottle was noted onsite without Drug identification Number. Operator was advised to obtain a low level disinfectant with DIN.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Reprocessing area, sink was noted in unsanitary condition. Spill of hair color was noted on the surfaces. Non sanitizable mats were noted under the drying rack of washed equipment. Operator was advised to follow the decontamination procedure of dual-purpose sink to clean the sink and surrounding area and maintain in a clean and sanitary condition.
