- Daily testing results for High-Level Disinfectant (HLD) were inadequately maintained. The correct procedure for entering daily test results in the logs was reviewed with the owner.Ensure that daily HLD test results are adequately entered and maintained in logs.

- The owner demonstrated inadequate reprocessing of nail drill bits by skipping the pre-disinfection cleaning and post disinfection rinsing steps and failing to completely submerge the drill bits in the disinfection solution.The correct procedure for reprocessing semi critical instruments with HLD were reviewed with the owner.Ensure that proper reprocessing steps are followed.

- No puncture resistant sharps container was noted in the facility. Improper disposal of sharps in a plastic container was noted. The owner was advised to obtain a puncture resistance sharps container and implement safe sharps disposal practice.Obtain a puncture resistance sharps container and ensure safe disposal of sharps.

- Disinfection of the drill bits was reportedly done at the client service station by each operator. A spray bottle of high-level disinfectant was stored at each client service station. High level disinfectant bottles were removed from the service stations. Another high-level disinfectant container for immersing the equipment was noted at the waxing station. All disinfectant containers were move to the reprocessing area and operator was advised to store high level disinfectant at the reprocessing area to do all reprocessing in the reprocessing station. Ensure that all the cleaning and disinfection of the semi-critical equipment is conducted at the designated reprocessing area.

- 14. Is single-use or uncleanable equipment being used as intended (Non-Critical)?
- Dirty single-use buffer blocks and disposable sanding bands for nail drills were observed in nail station cabinet on top of other clean equipment. Personal service worker indicated that they had overlooked discharging these items after it was used on a client.Reusing porous or uncleanable tools is unacceptable. Train personal service workers to immediately discard single-use tools used on clients should be discarded after each use.

- Residues from cosmetic products used during pedicure were observed on the pedicure bowls. Pedicure bowls were not adequately scrubbed after each client. Bowls were disinfected using a bleach solution. - Clean all pedicure bowls thoroughly to remove residue. Bleach solution is not permitted as disinfectants unless it has a Drug Identification Number (DIN) and have clear manufacturer’s instructions for use on the appropriate classification of reusable equipment.

- Debris observed in parafin wax tub. Surfaces of tub used for warming parafin wax was covered in wax.Discard wax and clean tub thoroughly to remove wax residue.

- 1) The nail station surfaces, drawers and trolleys had buildup of nail debris, dust and nail polish residue. Surfaces was cleaned thoroughly during inspection.- Ensure that all work surfaces are thoroughly cleaned and maintained in sanitary condition at all times.2) The middle pathway floors were slippery due to nail dust. Floor was cleaned during inspection. - Clean the floor thoroughly.

- 1) Debris accumulated inside the plastic cabinet used for storing with acrylic nails.2) Surfaces of the wooden storage cabinet beside the reprocessing station, used for extra supplies was unclean and disorganized. - Cabinet surfaces were cleaned during inspection.Ensure items in storage cabinets and other areas are regularly cleaned and re-organized.

- Semi-critical tools including foot files was immersed in an unapproved disinfectant for personal services tools. Semi-critical tools must be reprocessed using a disinfectant with a drug identification number (DIN), a general disinfectant claim, a tuberculocidal claim and specifically states high-level disinfectant, chemical sterilant or sporicidal after cleaning.

- 05. Are written procedures reviewed and revised when required, and do they meet the requirements set out in the standards?
- The written procedure for the reprocessing of reusable equipment does not adequately describe for the required steps. Used semi-critical and critical equipment must be:- disassembled to its simplest component parts for cleaning; - cleaned to remove soil, and rinsed to remove dislodged soil and detergent residue; and- dried prior to disinfection or sterilization. - cleaned equipment must be visually inspected for cleanliness prior to disinfection- immersed in disinfectant for the required contact time indicated by the manufacturer's label- rinse equipment thoroughly and- air-dry on a clean towel.Update the reprocessing steps instructions as outlined above. Submit it to the inspector for review prior to posting.
