      • 3-Junior Pre-school room, Shelving unit and table were noted with worn out paint. All surfaces of the structure and furniture are required to be smooth and wipeable to prevent harboring of germs.

      • 1-Water pressure and quantity at both hand washing sinks in the employee washroom were in sufficient for proper hand washing. Repair required.Ensure hand washing stations are fully functional and equipped with supplies all times.

      • Junior Pre-school Room - Diapering mat surface was noted with rips resulting in non-wipeable and sanitizeable surfaces. Replace the mat or install a waterproof cover on it.2-Napping room - diapering table located by the hand wash station was noted with a crack in the structure. Fill the crack and ensure the surface is smooth to be wipe down adequately.3-Junior Pre-school room, Shelving unit and table were noted with worn out paint. All surfaces of the structure and furniture are required to be smooth and wipeable to prevent harboring of germs.

      • 1-Water pressure and quantity at both hand washing sinks in the employee washroom were in sufficient for proper hand washing. Repair required.Ensure hand washing stations are fully functional and equipped with supplies all times.2-Paper towel roles were noted not being stored in the dispensers at the handwashing stations. Paper towel roles were places on the shelves where these can get wet and contaminated. Ensure all paper towels are stored properly.

      • Indoor play area- A climbing structure with an inappropriate slide was noted. The slide was removable with no canopy, sitting platform and landing space at the exit. The slide was removed during inspection.

      • 1-Sensory room- Insufficient surfacing noted by the climbing wall. The area up to the door is recommended to be covered with protective surfacing to prevent injuries in case of falls.2-Climbing structure in the indoor playground- A climbing structure with removable slide was noted with insufficient fall zone. Relocate the structure to provide sufficient fall zone around it.

      • Washroom with Laundry- The cabinet with chemical supplies was noted with out lock and child using the washroom unsupervised. Operator was reminded that the laundry area is required to be inaccessible to the children. Considering the situation, the use of washroom by children must be supervised and chemical cabinet must be lock all the time.

    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Rocks/stones were noted in the indoor playground that children can lift and drop on their own feet, the feet of other children playing nearby, or throw at other children, which could result in serious injuries.Remove all rocks/stones that children can pick up and throw.December 2, 2024- Discussed ways with operator to reduce hazards associated with the size of the stones/rocks. The operator was advised to replace the rocks with ones that are either large enough to prevent children from picking them up or small enough that they cannot be dropped unintentionally.

    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • 2-Temporary handwashing sinks were noted in the diapering areas of the infants' and toddlers' rooms. The water tank for the temporary hand sink was found to be empty during the inspection.Remove the temporary handwashing stations and take children to the bathroom for diapering (as being primarily approved) or install plumbed sink in each room with diapering area. A sink with running hot and cold water, soap, and paper towels must be accessible in each diapering area.

      • 1. Rocks/stones were noted in the indoor playground that children can lift and drop on their own feet, the feet of other children playing nearby, or throw at other children, which could result in serious injuries.Remove all rocks/stones that children can pick up and throw.2-A wood table was noted in the indoor playground area with rough surfaces and exposed studs, posing a risk of injury to children's fingers.3-Surfaces of the wooden table in play area were not smooth, cleanable, wipeable.Ensure the surfaces are smooth, non-absorbent, smooth, cleanable, wipeable and in good condition.4-Insufficient turf/protective surfacing to cover the fall zone around the equipment with height above 20 inches.

      • 1-. Reusable dishes were noted during the inspection as being provided to children. However, there is only a single sink in the staff room. A 2-compartment sink to use reusable dishes in the facility.Only provide single-use disposable dishes to the children or replace the single sink with a 2-compartment sink. or serve food to the children in their lunch boxes and send them home to be washed.2-A patch on the floor near the main playground area was noted to be covered with duct tape. It is reportedly covering an electrical panel. The area covered with duct tape is not cleanable, wipeable, or sanitizable. The duct tape should be removed, and the electrical panel should be properly capped to prevent access by children. The area should also be made cleanable and sanitizable.

The operator was advised to provide either paper towels or single-use microfiber towels, along with a basket beside the sink to store used microfiber towels.2-Temporary handwashing sinks were noted in the diapering areas of the infants' and toddlers' rooms. The water tank for the temporary hand sink was found to be empty during the inspection.Remove the temporary handwashing stations and take children to the bathroom for diapering (as being primarily approved) or install plumbed sink in each room with diapering area. A sink with running hot and cold water, soap, and paper towels must be accessible in each diapering area.

      • 1-A rebounder was observed in the room located at the back of the facility, Remove the rebounder.2-A swing made of rope was noted inside the play structure in the kindergarten room posing entanglement hazard.The operator was advised to remove the swing from the play structure during the onsite inspection.3-A mix of swings and climbers was observed in the hallway of the indoor playground. All structures were attached with a single strap tied to the poles, making them unstable. Open S hooks were noted.Remove the swings, climbers, and hangers from the hallway and have them installed professionally using appropriate materials with protective surfacing.4-There was no fall protective surfacing under the climbing equipment. Measure the fall height of all climbing and slides and install appropriate fall protective surfacing under all climbing structures, slides and swings with fall height more than 20".elevated equipment is defined as equipment which has a fall height of greater than 500 mm (20 in). Equipment with a fall height of less than or equal to 500 mm (20 in) should still belocated on a surface that does not pose a safety concern (e.g., not on cement).5-The slides in the indoor play area were not properly secured to the equipment.Secure the slide to the equipment to prevent fall incidents.

      • 1-The climbing wall structure was located in the play area next to the storage room, but there was insufficient fall zone on the east side by the door.Extend protective surfacing to cover the area on the east side of the equipment.2-A wooden table with rusted screws and porous, non-smooth surfaces was noted in the reportedly newly developed indoor play area. All surfaces in the facility need to be wipeable, cleanable, impervious to moisture, and in good repair.Ensure that all furniture surfaces in the facility are cleanable, wipeable, smooth, impervious to moisture, and in good repair.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • The faucets of the handwashing sinks in the unisex bathroom located on the southeast side of the facility were not properly secured. One of the hand sinks was not operational during the onsite inspection. Water supply was turned off at the time of inspection. Secure the faucets and repair the handwashing sink. Ensue hand washing sinks are operational and adequately supplies with hot/cold running water, liquid soap and paper towels.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • Pre-packaged snacks and juices were observed to be stored on the ground in the storage room. Art supplies, chemicals, and food items were not stored separately.Store food items on the top shelf and ensure that non-food items such as art supplies and chemicals are stored separately on different shelves or units. All storage must be at least 6 inches off the floor to facilitate cleaning.
