    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Two small, upholstered sofas used by children were observed with unknown stains at the time of inspection. Upholstered furniture that is visibly soiled may harbor microorganisms and can contribute to the spread of illness if not cleaned and maintained in a sanitary condition. Ensure that the sofas are thoroughly cleaned using an appropriate method, such as steam cleaning, and maintained in a clean and sanitary condition at all times.
    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • At the time of inspection, only cold water was available from all handwashing faucets throughout the facility, which does not meet the requirement for handwashing facilities to provide hot and cold running water. This was discussed with staff during the inspection. CDI: Staff adjusted the water temperature during the inspection. Hot and cold running water was subsequently available at all handwashing sinks, and this was verified. Ensure all handwashing facilities are maintained in proper operating conditions and continue to provide an adequate supply of hot and cold running water.
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • Some ant activity was observed near the exit gate in a children's classroom at the time of inspection. The presence of pests may result in the contamination of surfaces, equipment, toys, and other items within the facility, and indicates that effective pest control measures are not being maintained. Ensure effective pest control measures are implemented and maintained to eliminate pest activity and prevent the harborage and entry of pests into the facility.

    • 05. Are communicable diseases at this facility being properly managed?
      • One child's room was not provided with bleach solution at 1000 ppm and only had 100 ppm solution for use on surfaces. Operator was arranging for another container of solution to be provided. Soft items and toys that can't be washed and sanitized between clients were still in use. Staff were advised to remove soft items and any toys that cannot be disinfected until the outbreak is declared over. Ensure disinfectant is used on high touch surfaces frequently throughout the day.

      • The facility is experiencing an outbreak and no signage was posted at the entrance notifying people of the outbreak. Please ensure signage is posted in a location that is easily visible to those entering the facility.

    • 05. Are communicable diseases at this facility being properly managed?
      • Some staff indicated blood and body fluids would be disinfected with a 100 ppm bleach solution. Discussed proper concentration with staff. Please ensure staff are mixing bleach solution for blood and body fluid cleanup at 1 part bleach to 10 parts water.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Both sinks in the seekers room were obstructed with items left in them. This left no sink accessible for hand washing after diapering. Please ensure the sink next to the diaper change table is clear and accessible for hand washing at all times.

    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Staff member was observed not washing hands after glove use when changing a child's diaper and or helping a child use the washroom. When reminded that washing hands is important, staff member ran their hands in the sink for less than 5 seconds without using soap. Please ensure all staff members are washing their hands properly specially in between changing diapers, helping a child or using the washroom, before and after meal services, etc.
