      • Wooden tongue depressors were noted being re-used and stored in the food scrub container- ensure single use items are disposed after one use and that there is no double dipping

      • Test strips are not being used and no logs for the CS20 high level disinfectant were available - during the inspection PHI had operator test the prepared CS20 high level disinfectant and it was noted as a fail. New solution was prepared and re-tested providing a pass- ensure test strips for the CS20 high level disinfectant are used daily and recorded in a log
