- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- Operator did not inform the department that new services (laser, dermabrasion manicure, pedicure) were being provided.This issue was corrected during the inspection. Operator knows to contact our department should new services be added.
- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- A written procedure for decontaminating the handwashing sink was not developed.REQUIREMENT: Develop a written procedure for the above.

- Paper towel rolls at handwashing sinks were not attached to a dispenser/mount to protect the bulk supply.REQUIREMENT: Obtain and install a dispenser/mount at each handwashing sink.
- 20. Is reusable equipment disinfected properly?
- Operator was using Preempt CS20 to disinfect tools for 10-15 min instead of 20 minutes as required.Operator will reprocess the tools again to ensure 20 minutes was achieved.

- No user manual for the IPL Laser machine was readily available.REQUIREMENT: Obtain and ensure the user manual for the IPL Laser machine remains readily available. Ensure the machine is used in accordance with the manufacturer's instructions.
- 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
- Operator had a reprocessing sink for semi-critical tools located in the washroom (contains a flush toilet). This reprocessing sink was changed to a sink located in a procedure room.
- 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
- The facial steamer had a notable amount of scale on the element.REQUIREMENT: Descale the facial steamer.
