    • 06. Are proper protocols being followed in regards to client care?
      • 1. Bleach solution was not available by the diapering area in the toddler room. Please ensure the bleach solution is available by diapering at all times.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Toddler Room & Baby Room: The wooden countertop by the sink is water‑damaged. Please repair or replace it so the surface is non‑porous, smooth, and easily cleanable.

    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. All Classrooms: Several push pins were observed in use. Please remove all push pins, as they may fall onto the floor and pose an injury risk to children. Use tape or sticky tack as safer alternatives.2. Preschool & Toddler Room: Sensory bottles were easily openable. Please seal them securely to ensure they cannot be opened by children.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Baby Room: An appropriate disinfectant was not available at the diapering area. Please ensure a 100 ppm bleach solution is accessible at all diapering stations. The Clorox wipes currently available do not indicate effectiveness against norovirus; please replace them with the required bleach solution.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Toddler Room & Baby Room: The wooden countertop by the sink is water‑damaged. Please repair or replace it so the surface is non‑porous, smooth, and easily cleanable.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • 1. Baby Room: The bleach solution was found to be mixed with soap. Please ensure staff are trained on how to properly prepare bleach solutions. Bleach must never be mixed with soap, as this reduces its effectiveness and may create unsafe chemical reactions. Only water and bleach should be used when preparing the required 100 ppm solution.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • 1. Baby Room: Foam blocks with bite marks were observed and are no longer cleanable. Please replace these damaged toys..2. Baby Room: A mouthed‑toy bin was not available. Please ensure a designated bin is provided for separating mouthed/dirty toys.3. Baby room: A cracker packet was stored next to visibly dirty slime. Please discard used or uncleanable play materials such as slime. A staff member indicated the cracker belonged to the staff; please ensure all staff personal belongings are stored separately and away from children’s materials.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. Toddler Room & Baby Room: Some children’s blankets were stored on top of cots and were in contact with one another. Please ensure all blankets are stored separately to prevent cross‑contamination.
    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • 1. Toddler Room & Baby Room: Used cleaning rags were found by the sink or placed on top of spray bottles. Please ensure the following:- Cleaning rags are used once only, then either disposed of or laundered to prevent bacterial spread.- Each classroom must have a clearly labeled bin designated for used rags.
