- Observation (CORRECTED DURING INSPECTION): Bleach sanitizer not prepared during inspection. Staff on site prepared bleach sanitizer at a concentration of 200 ppm.
- Corrective Action(s): Prepare bleach sanitizer solution in the morning at a concentration of 100-200 ppm by mixing 1/2 to 1 teaspoon of bleach with 1L of water to adequately sanitize food contact surfaces throughout the day.

- Observation (CORRECTED DURING INSPECTION): Three boxes of produce observed stored directly on the floor in the food storage area.
- Corrective Action(s): Move all boxes to shelves which are at least 6 inches high. Do not store boxes of food directly on the floor to prevent contamination. All food must be stored covered and at least 6 inches off the floor.

- 1. One live cockroach/bug observed on site in hand washing station basin at the front service area.
- 2. Sticky traps observed with cockroaches/bugs. Professional pest control company is contracted.
- Corrective Action(s): Contact pest control company to assess habrouring/breeding areas and apply treatment/traps as necessary to remove current population of cockroaches/bugs. Maintain the facility in a clean and sanitary condition to prevent future habouring/breeding. Keep all food covered and stored at least 6 inches off the floor to prevent potential contamination.

- 1. Do not store dirty mop water. Discard mop water after each use to prevent pest entry/harbourage/breeding.

- Corrective Action(s): Degrease, clean and sanitize aforementioned areas to remove grease and food debris. Facilitate frequent cleaning and sanitation of the facility to ensure it is maintained in sanitary condition and to prevent pest entry/harbourage/breeding on site.
