      • The Cavicide disinfectant spray used to disinfect non-critical equipment and client surfaces was expired.- New Cavicide disinfectant was ordered during the inspection and Clorox disinfectant will be used until the Cavicide arrives.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • There was debris and cobwebs noted on the reprocessing counter used to air dry and package tools for sterilization.- Area to be cleaned and disinfected.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Open boxes of sterilization pouches over the reprocessing area had dust/debris and cobwebs.- Sterilization pouches and related equipment shall be protected from cross-contamination.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Several critical tools being soaked in the reprocessing sink were rusting.- Ensure rusted equipment is discarded.
