      • Baby Room: Peeling paint was observed on the wall, and damage was noted on the corner of the wall near the crib. This condition presents a safety risk, as infants may pick at and swallow loose paint or wall material while playing, creating a choking hazard and potential exposure to harmful substances. The operator was advised to repair the wall to prevent risk and ensure a safe environment for infants.

The operator was advised to ensure that the required cleaning has been completed to support a safe and healthy environment.

      • Baby Room: Cleaning chemicals were observed stored in an unlocked cabinet under the sink in the baby room, with the baby dining area located in close proximity. Staff were advised to secure and lock the cabinet to ensure chemicals are inaccessible to children and to prevent any potential chemical hazards. The drawers were subsequently locked by staff upon request.

      • Baby Room: In the baby room, spacing between beds was observed to be 8 cm and 12 cm, which did not meet required guidelines. Staff were advised that sleeping areas must be arranged to provide a minimum of 60 cm of open space on all sides of each bed, except where a bed is positioned directly against a wall. Staff subsequently moved the beds to achieve appropriate spacing and ensure compliance.

      • Baby Room: Unlabeled milk bottles were observed stored together in a shared container in the baby room, creating a risk of mix‑up. Staff were advised to clearly label all bottles and items for each child to ensure proper identification and to maintain children’s health and safety.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Sensory Playroom: The floor was observed with sand, dust, and debris, particularly accumulated under the rug. The operator was advised to clean the floor to maintain a clean and safe environment.

    • 08. Is this facility properly constructed (Non-Critical)?
      • Facility has replaced all ceiling tiles since last visit from EPH in July, and yet there are multiple water stains in various rooms again. There is evidently an issue with the roof, which is not the responsibility of the facility, but rather the property management. Changing the ceiling tiles continuously will not address the root cause, and leaving this issue will only lead to rot and mould.

    • 06. Are proper protocols being followed in regards to client care?
      • During a declared outbreak, facility was using approximately 500 ppm chlorine bleach sanitizing solution vs. the required 1000 ppm solution. This was due to infrequent remaking of solution and user error with difficult to read test strips. All sanitizing solutions were increased at the request of the PHI and new test strips that are easier to read were ordered.

      • Facility has replaced all ceiling tiles since last visit from EPH in July, and yet there are multiple water stains in various rooms again.       • The measured temperature at the Toddler bathroom sink was 53.7 C. Please adjust the temperature to be in line with the recommended value of 49C.

      • The Kinder/OSC washroom has a hole in the wall from the door handle being opened too far. Please repair. A suggestion to install a commercial doorstop on the floor was made, to prevent this situation from reoccuring.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • There is leftover adhesive on the floor in several of the rooms. Please use an adhesive remover to clean the floors properly and to allow for proper cleaning moving forward.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • The measured temperature at the Toddler bathroom sink was 53.7 C. Please adjust the temperature to be in line with the recommended value of 49C.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • A wet cleaning cloth in use was observed sitting out on the counter in the Sr. Toddler Room. Cloths are to be used once and put into dirty laundry or kept in a sanitizing solution when not in use.

      • Oct 27: The operator installed an acceptable temporary physical barrier to cover the missing tile in the kinder room ceiling. A permanent solution must be found to repair the hole in the ceiling tile and ensure the ceiling is smooth, impervious to moisture and easy to clean.Oct 19: There was a water leak from the pipes above the kinder room and the ceiling tile was missing due to ongoing repairs.

      • The toilet paper rolls in the preschool bathroom were being stored on top of the dispenser instead of inside.
