- Staff members’ personal items, including a phone, water bottle, jacket, and beverage cup, were observed on the countertop in a food handling areas.The operator was advised that all personal belongings must be stored in designated staff storage areas and not in food preparation or service areas. Operator was also reminded not to consume food or beverages while preparing or serving food.Food and drink may only be consumed in designated staff or dining areas. Staff must wash their hands thoroughly before returning to the kitchen to ensure proper food safety practices are maintained.

- The hot water line at the handwashing sink was observed to be leaking. The operator was instructed to repair the issue immediately, as it prevents staff from properly washing their hands. In the interim, staff are using the handwashing sink located at the front of the facility.

- A white cutting board was observed with deep grooves and accumulated black debris, indicating it is no longer easily cleanable.The operator was instructed to either resurface (repair) or replace the cutting board to ensure it can be properly cleaned and sanitized, in compliance with food safety standards.

These conditions indicate inadequate cleaning and maintenance, and pose a potential risk of contamination to food and food-contact surfaces. The facility is not being maintained in a clean and sanitary condition as required.Operator was advised to thoroughly clean and degrease all affected ceiling areas and ventilation fixtures. Maintain ceilings in a clean condition on a routine basis.

- Knives were stored in between the covers of the prep cooler. The knives were removed, cleaned and sanitized upon request. Ensure knives are stored in a manner that protects them from contamination.

- Flooring tiles under the grill was observed to be broken and in disrepair. Repair/refinish the flooring and ensure it is durable, smooth, and impervious to moisture to facilitate ease of cleaning.

- Dirt and grease build up was observed along the floor in the bar area, under the cook line, and under the dishwasher. Dust build up was also observed in the walk-in cooler and walk-in freezer fan covers. Clean the above mentioned areas.

- Multiple cleaning cloths were observed in use and on counters instead of in an approved sanitizer solution.

- Over 20 houseflies were observed during inspection, many of them landing on the soiled section of dining room carpet, most likely eating whatever had backed up onto it.

- There are holes to the right of the mop sink and on the other side of the wall to the left of the mop sink that must be repaired.

- There has been a back up of some sort of septic water onto the floor and the dining room carpet. All these areas must be cleaned. Floors throughout the kitchen require deep cleaning with special attention to corners and underneath equipment. Carpets must be cleaned and if they cannot be, they must be replaced.
