- Observation: In the dry goods storage room, the cover on the ceiling light fixture is broken. If fluorescent light tubes drop from the fixture, they might fall and break, spilling glass pieces on the stored items and floor.
- Corrective Action: Install a new cover on the light fixture.

- Observation: (CORRECTED DURING INSPECTION) The concentration of QUATS in sanitizing buckets is inadequate @ 100 ppm. A concentration of at least 200 ppm is required.
- Corrective Action: Maintain QUATS concentrations at 200 ppm or more.

- Observation: Floor surfaces are dirty in the drive-thru window area, near garbage receptacle under counter (dirt and wasted paper goods) and custodial room (broken glass).
- Corrective Action: Clean the floor thoroughly and maintain better floor cleaning throughout the facility.

- Observation: (CORRECTED DURING INSPECTION) Employee clothing is stored on storage shelves near coolers in backroom. This may contaminate items.
- Corrective Action: Store employee clothing in designated areas.

- Observation: In the griller machine area, the tray containing spatulas is located beside the griller machine and under the paper towel dispenser. The spatulas are subject to contamination from water and/ or soap dripping from employee hands.
- Corrective Action: Relocate the tray containing the spatulas to location where the spatulas are not subject to contamination or provide hooks to hang the spatulas in a safe location.

- Observation: Cardboard boxes of paper supplies stored near ovens in the back room are soiled with grease from oven gloves stores on them. The boxes are contaminated.
- Corrective Action: Remove the affected cardboard boxes and provide storage of oven gloves so that boxes are not contaminated with grease.

- Observation: Ice forming at the condenser and on food product boxes underneath it inside the freezer. Some of the boxes are left open with food products exposed. Ice may contaminate food products and increase risk of foodborne illness.
- Corrective Action: Operator to repair condenser in freezer. Operator to move boxes underneath condenser while it is awaiting repairs. Food products should be covered and protected from contamination when it is being stored in the freezer.
