      • High-level disinfectant test strips were observed to be expired as of February 2026. The operator was advised to purchase new test strips. Follow-up will be conducted to verify that this has been completed.

      • Expired PreEmpt disinfectant was observed during the inspection. PreEmpt is not considered a suitable high-level disinfectant. The facility did have non-expired CS20, and the operator was advised to use this high-level solution for disinfecting tools. Please ensure that non-expired high-level disinfectant is being used on re-usable, semi-critical tools at all times.

      • Please inform staff to read manufactures labels for contact time of sanitizers and disinfectants. When questions staff member who does the cleaning, it was observe that contact time of certain products are not being followed.

      • Ensure High-level Disinfection are being tested every time in use to ensure efficiency, please also record date when solution is changed.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Please ensure staff members are not eating in procedure areas with or without clientele.
