      • At time of inspection:1. A discussion was conducted with the owner of this building. He has indicated that the roof was in the midst of being repaired / replaced. From photographs shown, it was indicated that the roof was needing to be repaired / replaced.The owner / operator of this facility is requested to conduct the following, namely:1. Contact the acting AHS Executive Officer once the roof has been fully repaired / replaced. Please submit any receipts for this work being completed.*****AT TIME OF INSPECTION, THIS ISSUE REMAINS PENDING COMPLETION.       • At time of inspection:1. One infant room (next to the front desk) contained an infant change table and a soiled garbage container in close proximity to each other. There were also items stored right next to this infant change table where infants can touch / handle.The owner / operator of this facility is requested to conduct the following, namely:1. Do not store any garbage containers or equivalent (with soiled diapers within) next to where infants are being changed. All soiled diapers (and anything related) must be kept separate from any clean areas. 2. Re-organize both infant rooms to ensure all creams etc. are properly stored in a nearby cabinet. All soiled diapers must be discarded into a suitable garbage receptacle where infants do not have access to this area.

      • At time of site-visit, the following items were discussed:1. A non-porous infant changing mat was allegedly inserted on top of a counter in one of the infant childcare rooms. The director indicated that she stopped this practice once she found out about it. 2. Personal beverages (possibly hot beverages) were being stored near infant change tables.3. Staff were not following the AHS procedures for diaper changing.It was recommended that the owner / director conduct the following, namely:1. Discuss the diaper changing station issue with the staff in this childcare room. Determine what the staff would like to see happen to help alleviate from any time constraints involved when meal periods are about to occur. If a second change table is proposed to be installed in this room, the same type of change table can be installed as long as it:- is secured to the wall- is close to a hand wash sink - is reasonably away from where children consume food.2. Find a suitable location to where all staff beverages can be safely stored.3. Ensure that all staff are following the AHS guidelines for diaper changing.

      • At time of site-visit, the following items were discussed:1. Fruit was allegedly not being processed (cut) into small enough pieces for older infants to properly chew and swallow. Recommendation:1. Discuss this issue with the kitchen staff to ensure that fruit (or any food items meant for older infants) are cut at a more suitable size to properly chew and swallow. Site staff within any childcare room should not have to handle / cut food into smaller portions for older infants to consume. All food handling must occur within the commercial kitchen situated within this childcare facility.

    • 21. Is the plumbing system adequate and being properly maintained (Critical)?
      • Nova Room: Hand washing sink - a hose is connected below the sink which runs along the wall and then goes through the wall to be drained outside on the lawn. P-trap is also missing as gravity is used for drainage. The drainage system is not installed properly and according to the plumbing code. Contact a certified plumber and make necessary repairs immediately.

      • Mechanical room: There is a hole on the exterior wall. This was created to run a garden hose from the mechanical room to the outside for the purpose of watering the lawn. Ensure a proper exterior faucet is installed for the garden hose. This should be done according to the plumbing code. Contact a certified plumber and make necessary repairs immediately. If not, then seal the hole to prevent entry of pest.

      • At time of inspection:1. The director of this facility has indicated that they have already purchased two (2) sanitary mats (that are non-porous to moisture and easy to clean) in order to change diapers properly while laying down (i.e. when a child needs to be changed close to the floor surface). It was indicated that children will no longer be placed directly onto any floor surface (i.e. laying on the floor or equivalent - directly) in order to have their diapers changed. *****THEREFORE, THIS ISSUE HAS BEEN ADDRESSED.

      • There are internet outlets (not use) that do not have child proof covers.Ensure internet outlets not in use have a blank no device wall plate.

      • Storage room under the stairs: door is not lockable. Ensure door is lockable and kept locked at all times to prevent client access.

      • Cabinets underneath the hand-washing sinks in client rooms do not have child safety locks. Ensure child safety locks are installed on all cabinets underneath the hand-washing sink.

      • There are a few fire exit doors located inside client rooms. These doors are not locked, and clients can easily access restricted/hazardous areas.Consult with the Fire Department and find alternative locking devices that will be acceptable.

      • Water temperature is either cold or too hot at the hand-washing sinks inside the facility. Ensure all water temperature is adjusted at the handwashing so that is lukewarm.

      • Accessible washroom: Running hot and cold water is not available at the hand-washing sink.Ensure faucet is repaired.
