- Written procedures were not available at the facility for skin cleansing (for waxing services), accidental cuts during haircutting services, or for sink decontamination when sinks are used for both hand-washing and for washing tools or hair. Corrective Action: Written procedure templates have been emailed to the manager. Please fill the procedure templates out, follow the procedures, and display these documents where staff and visiting inspectors can view them.

- Written procedures were not provided at the time of inspection for skin cleansing before services such as waxing.Corrective Action: Please fill out written procedure templates, display them for staff, and follow them accordingly.
- 20. Is reusable equipment disinfected properly?
- Reusable equipment is disinfected properly with the exception of the high-level disinfectant (PREempt HLD5) which is used to disinfect straight razor handles. At the time of inspection, this disinfectant is not being tested for concentration, and a log-sheet was not available to track this testing.Corrective Action: This high-level disinfectant must be tested continuously with AHP2 test strips, and this must be logged with a log sheet. Alternatively, a low-level disinfectant, such as one used for other reusable hair tools (scissors, combs, etc.), can instead be used to disinfect straight razor handles without the testing and logging requirement.

- Written procedures were not available for cleansing of the skin and mucous membranes, client care in the event of accidental skin cut or puncture, and sink decontamination for a dual purpose sink. - Ensure required written procedures are completed and available onsite. Written procedures were completed by the operator during the inspection.

- A clipper stored in a drawer at a client service station was noted to have hair debris on the comb attachment. - Ensure to adequately clean reusable equipment with a visible inspection for cleanliness prior to disinfecting clippers after each client use.
- 20. Is reusable equipment disinfected properly?
- Debris build up was noted at the bottom of a Barbicide solution container at one of the client service areas. The operator reported changing the Barbicide solution approximately every two days. - Ensure to follow manufacturer's instructions for use of Barbicide disinfectant, which requires preparing a fresh solution daily or more often if the solution becomes diluted or soiled. A fresh Barbicide solution was prepared by the operator.
