      • A ceiling tile was observed misplaced due to facility maintenance. Facility operator stated the tile had been like that for about a week.Place the ceiling tile back into place to prevent food contamination.

      • The paper towel for hand hygiene was stored on the eye wash station and the paper towel dispenser was empty. Ensure that the hand sink is properly stocked with paper towel that does not impede on the eye wash station.

      • The concentration of the quat sanitizer in the kitchen was measured at 400ppm.Operator diluted the sanitizer solution and was measured at 200ppm.Ensure adequate concentration of sanitizer solutions are maintained. Quat solutions should be at 200ppm.

      • Food items consisting of bananas, cranberry juice, and hot sauce were stored on the kitchen floor. Facility operator placed a milk crate under the food items. Ensure all foods items are stored above the floor to protect them from contamination.
    • 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
      • The sanitizer concentration in the dishwasher was measured at 50ppm. Investigate and service the dishwasher, ensure that the dishwasher adequately dispenses the proper sanitizer concentration. Until dishwasher is serviced, it was discussed the 3-compartment sink method will be used.

      • The paper towel for hand hygiene was stored on the eye wash station and the paper towel dispenser was empty. Ensure that the hand sink is properly stocked with paper towel that does not impede on the eye wash station. Note: Facility cannot find the properly sized paper towel for the dispenser, it is recommended to change the dispenser.

      • Building material on the wall behind the 3-compartment sink was exposed. Repair/refurnish the wall to a smooth, durable, and moisture resistant surface to prevent mold and bacterial growth.
