      • 1. The wall on the left side of the stainless steel shelving in the kitchen was observed with a surface that is not smooth, durable, and easily cleanable (exposed drywall). Refinish or repair the wall to ensure the surface is smooth, durable, and easy to clean.2. The wall vent cover in the same area mentioned above was in disrepair. Repair / fix the vent cover.

      • Quat and Bleach sanitizer test strips were expired. Obtain new non-expired test strips. Note: PHI tested and compared the facility test strips against the PHI's test strips. Facility test strips were observed to be working. Facility staff were instructed to continue using the old test strips until the new non-expired ones are obtained.

      • The chlorine sanitizing concentration of the dishwasher was measured at 0ppm.Investigate and ensure the dishwasher sanitizing chorine concentration is measured at 100ppm.Note: it was discussed, single use utensils and the 2-sink sanitizing method will be used until the dishwasher can adequately sanitize.
