- Corrective Action: Operator post permit in a conspicuous location on premises.

- Observation: (CORRECTED DURING INSPECTION) Raw meat is stored above ready to eat foods and produce. Contact with and dripping fluids from raw foods can contaminate other items and lead to increase risk of foodborne illness.
- Corrective Action: Store raw meats below and isolated from ready to eat items and produce. Please ensure you follow proper storage method for your walk-in cooler.

- Observation: Wiping cloths left on the counters between use. (Public Health significance) - Cloths once used can harbor and grow bacteria if left as used at room temperature.
- Corrective Action: Store wiping cloths in a sanitizing solution of 100 ppm chlorine or 200 ppm QUATs.

- Observation: Ice machine is visibly dirty inside. Wall along the wooden table area have visible dried food debris. Racks inside cooler are dirty and greasy to touch. Unsanitary kitchen may attract pest and increase risk of cross contamination.
- Corrective Action: Operator to clean ice machine, clean racks and the wall along the wooden table.

- Observation: A pack of cigarette is found at the back side of the kitchen sitting on a table where a rice cooker is. Items not required for operation should be store away from kitchen.

- Observation: Evidence of rats within food in dry storage area. (Public Health significance) - All foods on the premises must be wholesome and free and otherwise fit for human consumption. Unacceptable food items could cause food borne illness and become inadvertently added to other food.
- Corrective Action: Remove and dispose of all foods that may have been contaminated by rats, their urine, or feces from the premises.

- Observation: Food containers were left uncovered and open to contamination. (Public Health significance) - Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
- Corrective Action: Cover or otherwise protect all foods from airborne contaminants, pests, and contact with objects and staff.

- Observation: Significant evidence of rats; droppings, urine, greasy smears, and holes gnawed through the wall were noted in the facility. (Public Health Significance) Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
- Corrective Action: Eliminate access to the building and routes they use inside. Trap and remove the pests. The use of a professional pest control operator is recommended. Clean and sanitize areas where droppings or evidence of rats are located and monitor situation.

- Observation: Leaking pipes and accumulation of moisture under the handwash station and dishwasher. Sufficent clutter within the facility. General sanitation of facility also needs improvement. Public Health significance - Water and poor sanitation provides nutrients for pests, and can attract or worsen pest infestations. Clutter within a food facility provides areas for pests to live, breed, and hide.
- Corrective Action: Ensure that leaking pipes are fixed and that the facility is kept clean and sanitary to reduce rodent attractants. Organize storage and remove any items or equipment not required for food preparation.

- Observation: Walk-in freezer unit is warm at -4 C. Food is not compromised, but this can indicate future failure and inadequate freezing.
- Corrective Action: Operator to repair and service the freezer so it can reach -18 C.

- Observation: Dishwasher not adequetely dispensing chlorine solution required to sanitize (50-200 ppm).
- Corrective Action: Have the dishwasher repaired and serviced to dispense appropriate amount of sanitizer.

- Observation: (CORRECTED DURING INSPECTION) Mould and grime build up on walk-in refrigeration shelves. Bins storing tools also contained debris.
- Corrective Action: Clean and sanitize the shelves in the walk-in refrigerator and regularly clean bins storing equipment.
- 315 - Refrigeration units and hot holding equipment lack accurate thermometers [s. 19(2)]
- Observation: Walk in freezer measured at -4 C. The thermometer in the freezer shows -20 C, which is inaccurate.
- Corrective Action: Replace the thermometer in the freezer and regularly do temperature checks and maintain a temperature log to ensure it keeps food frozen.
