    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. Long tangling wires were noted in several classrooms.-Please tack down or keep these wires out of children reach to prevent entanglement risk.2. Protruding screws observed adjacent to the toilet in senior preschool.-Please install a suitable cover on the indicated screws.
    • 06. Are proper protocols being followed in regards to client care?
      • Diapering surface in baby room was observed with extraneous materials including toys and children's bibs.-Please remove all materials placed on or near the diapering surface to prevent contamination.-Also, ensure the toys and bibs removed from the diapering surface are cleaned and sanitized before use.2. There was no dedicated diapering station in toddler 2 room. All the children in the room were being diapered by placing mat on the floor in a shared washroom.-Please provide a dedicated diapering surface in the indicated classroom to ensure proper procedure.

      • Dirty ceiling vents were observed in the senior preschool.-Please clean the indicated vents to maintain sanitary condition.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Sensory bottles covered with dirty multiple layers of tapes were observed in senior preschool classroom.-Please seal sensory bottles with non-toxic glue.The use of multiple adhesive tapes makes the bottles difficult to clean and sanitize.

      • Washroom hot water temperature was measured at 27'C or lower in both junior and toddler 2 classrooms.-Please increase and maintain hot water temperature between 38 and 43'C to support proper hand washing practices.

      • Update February 4, 2025:- Shelf was screwed in but not secured to a stud in the wall. Nail already came loose and needs to be resecured again.1) Senior pre-school - first set of cubbies on the left after entry, wall fastener has detached, so cubbies are no longer secured to wall.-Re-secure the cubbies

      • 1) Senior pre-school - first set of cubbies on the left after entry, wall fastener has detached, so cubbies are no longer secured to wall.-Re-secure the cubbies2) Senior pre-school - one brand of tempera paint had an age marking of 5+. Some children in the room were younger than 5 years old.-Operator should recheck all arts and crafts supplies to ensure they are age appropriate

      • Junior Pre-school SW corner - multiple ceiling tiles have water staining.-Investigate the source of the leakage, abate the leak, and then replace tiles

      • 1) Junior Pre-school - two heating vent grills in reach of children were damaged (some of the slats were broken leaving sharp edges).-Replace. Operator may want to look for sturdier materials as this is not the first time this damage has occurred2) Laundry room - the gap between the dryer and the wall had an accumulation of debris.-Ensure the area is regularly cleaned3) Laundry room - there was an accumulation of old fridges, creating clutter.-Remove clutter from the facility as it can harbour pests

      • **Not reviewed offsite** One dead cockroach was found on glue board located under the handwashing sink in the infant room.Notes -Continue to monitor for pests-Keep all areas in the facility clean, free from moisture and remove any food source in the classroom that attract pest.

      • Cubby in Junior preschool room that were >4' tall was not secured to the wall.**Cubbies in all other rooms have been secured***-Secure all unstable equipment >4' to the wall using appropriate anchors to prevent them from falling.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • Observed broken panels in the wall vents located below the windows in the preschool classroom. Broken vent panels have sharp edges that are accessible to kids. ****Vents in all other areas of the facility has been fixed***Action taken.-Operator was asked to place a barrier to limit children's access to the vent -Please repair the vent panels and keep classroom free from any physical hazard.

      • Cleaned toys observed drying over a towel on the floor.- Please dry cleaned toys off the floor to prevent the risk of contamination and any moisture source that can encourage pest harborage.- PHI recommended getting a shelf for drying toys if necessary.

Unsecured and reachable wires/cords for wall mounted heater were observed in the classrooms. -Secure all wires and cords for wall mounted heater in all classrooms.

      • Temperature in the infant room measured at 17-18C. Action Taken-Temperature adjusted to 20C during inspection. -Keep temperature of all rooms within the childcare facility at of 20C or greater.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Spacing between beds in the junior preschool were less than <60cm, too close to each other. -Spacing between each child during napping must be >60cm apart.
