      • Cabinets with ineffective child safety locks were observed in several classrooms, including the Senior Preschool. Sanitizer bottles and other chemicals were stored in some of these cabinets.-Please install and maintain effective child safety locks on all cabinet doors to prevent children's access to hazardous materials.
    • 11. Are parent provided foods being properly handled and stored to ensure the safety of clients in this social care facility?
      • Parent-provided food items were observed being stored at room temperature in the Junior Preschool classroom.-Please ensure that parent-provided perishable foods, such as yogurt, cooked foods, and cut fruits or vegetables, are stored in a refrigerator at or below 4°C upon arrival. Foods that require reheating should be heated to at least 74°C before service and allowed to cool to a safe serving temperature before being provided to children.OR-Ensure that parent-provided foods and snacks are supplied in appropriate containers that can maintain safe temperatures until consumed.       • 1. Play mat with peeling leather was observed in Senior preschool classroom.-Please replace or repair to ensure smooth and easy to clean surface.2. The walls and sink area in the Senior Preschool washroom were observed to be dirty.-Please thoroughly clean the identified areas and maintain in sanitary condition.3. Peeling paints were observed in different areas and in several classrooms:*on doors and walls of Junior Preschool washroom;*on the column/pillar in the Toddler classroom;*on walls and diapering table in the T-1 classroom; and *on window ledge in Baby room.-Please repaint the identified areas to maintain good repair.

      • Some of the toy containers in the Senior Preschool were dirty.-Please thoroughly clean the toy containers and to always clean them together with the toys.

      • Ineffective caulking observed around the sink areas in Toddler and Baby classrooms.-Please apply new sealant around the indicated areas to prevent water infiltration, thoroughly clean the areas as well.

      • Long tangling wires observed in toddler 2 and Junior Preschool classrooms.-Please secure the wires using clips or cable ties to prevent entanglement and tripping hazards.
    • 06. Are proper protocols being followed in regards to client care?
      • 1. Torn and dirty diaper mat was observed in infant room.-Please replace the damaged diaper-changing mat and ensure that the diapering surface is cleaned and sanitized after each diaper change.2. Staff was observed using gloved hands to wash a child's hands following a diaper change and subsequently touching another child near the handwashing sink while still wearing the same gloves.-Please ensure that gloves are removed immediately after completing the diapering process and before assisting a child with handwashing. New gloves must be used for each diaper change as required, and hand hygiene must be performed after glove removal.Proper diapering procedures and glove-use practices were reviewed and discussed with staff and the daycare operator.

      • Unsuitable diaper mat was observed in toddler 1 classroom. The diaper mat has a layer that is made of absorbent material.-Please replace the diaper mat with one that have a smooth, non-absorbent and easy to clean surface.

      • Peeling paint was observed on the window ledge walls in toddler 1 classroom.-Please repaint the identified areas to ensure smooth, impervious to moisture and easy to clean surfaces.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • Dusty vents and walls were observed in all classrooms and facility's corridor.-Please clean these vents and walls to ensure sanitary conditions.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • The fabric/tent material of the house/kitchen play structure in the Toddler 1 classroom is visibly dirty.-Please clean the playhouse structure thoroughly to maintain sanitary conditions.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • Water pooling was observed behind most classroom handwashing sinks, with visible mould growth present.-Please clean and remove the existing mould growth and apply fresh silicone sealant to eliminate the narrow gap behind the handwashing sinks. This will help prevent water accumulation or stagnation, and future mould growth.

      • Children water bottles were noted with name tags.-Please label all children water bottles with their names to prevent mix up.

      • 1. Long tangling wires were noted in several classrooms.-Please tack down or keep these wires out of children reach to prevent entanglement risk.2. Protruding screws observed adjacent to the toilet in senior preschool.-Please install a suitable cover on the indicated screws.

      • Diapering surface in baby room was observed with extraneous materials including toys and children's bibs.-Please remove all materials placed on or near the diapering surface to prevent contamination.-Also, ensure the toys and bibs removed from the diapering surface are cleaned and sanitized before use.2. There was no dedicated diapering station in toddler 2 room. All the children in the room were being diapered by placing mat on the floor in a shared washroom.-Please provide a dedicated diapering surface in the indicated classroom to ensure proper procedure.

      • Dirty ceiling vents were observed in the senior preschool.-Please clean the indicated vents to maintain sanitary condition.
    • 20. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Non-Critical)?
      • Sensory bottles covered with dirty multiple layers of tapes were observed in senior preschool classroom.-Please seal sensory bottles with non-toxic glue.The use of multiple adhesive tapes makes the bottles difficult to clean and sanitize.

Some children in the room were younger than 5 years old.-Operator should recheck all arts and crafts supplies to ensure they are age appropriate

      • Junior Pre-school SW corner - multiple ceiling tiles have water staining.-Investigate the source of the leakage, abate the leak, and then replace tiles

      • 1) Junior Pre-school - two heating vent grills in reach of children were damaged (some of the slats were broken leaving sharp edges).-Replace. Operator may want to look for sturdier materials as this is not the first time this damage has occurred2) Laundry room - the gap between the dryer and the wall had an accumulation of debris.-Ensure the area is regularly cleaned3) Laundry room - there was an accumulation of old fridges, creating clutter.-Remove clutter from the facility as it can harbour pests

Broken vent panels have sharp edges that are accessible to kids. ****Vents in all other areas of the facility has been fixed***Action taken.-Operator was asked to place a barrier to limit children's access to the vent -Please repair the vent panels and keep classroom free from any physical hazard.

      • Cleaned toys observed drying over a towel on the floor.- Please dry cleaned toys off the floor to prevent the risk of contamination and any moisture source that can encourage pest harborage.- PHI recommended getting a shelf for drying toys if necessary.

      • Temperature in the infant room measured at 17-18C. Action Taken-Temperature adjusted to 20C during inspection. -Keep temperature of all rooms within the childcare facility at of 20C or greater.

      • Spacing between beds in the junior preschool were less than <60cm, too close to each other. -Spacing between each child during napping must be >60cm apart.
