- 1. Long tangling wires were noted in several classrooms.-Please tack down or keep these wires out of children reach to prevent entanglement risk.2. Protruding screws observed adjacent to the toilet in senior preschool.-Please install a suitable cover on the indicated screws.

- Diapering surface in baby room was observed with extraneous materials including toys and children's bibs.-Please remove all materials placed on or near the diapering surface to prevent contamination.-Also, ensure the toys and bibs removed from the diapering surface are cleaned and sanitized before use.2. There was no dedicated diapering station in toddler 2 room. All the children in the room were being diapered by placing mat on the floor in a shared washroom.-Please provide a dedicated diapering surface in the indicated classroom to ensure proper procedure.

- Sensory bottles covered with dirty multiple layers of tapes were observed in senior preschool classroom.-Please seal sensory bottles with non-toxic glue.The use of multiple adhesive tapes makes the bottles difficult to clean and sanitize.

Nail already came loose and needs to be resecured again.1) Senior pre-school - first set of cubbies on the left after entry, wall fastener has detached, so cubbies are no longer secured to wall.-Re-secure the cubbies

- 1) Senior pre-school - first set of cubbies on the left after entry, wall fastener has detached, so cubbies are no longer secured to wall.-Re-secure the cubbies2) Senior pre-school - one brand of tempera paint had an age marking of 5+. Some children in the room were younger than 5 years old.-Operator should recheck all arts and crafts supplies to ensure they are age appropriate

- 1) Junior Pre-school - two heating vent grills in reach of children were damaged (some of the slats were broken leaving sharp edges).-Replace. Operator may want to look for sturdier materials as this is not the first time this damage has occurred2) Laundry room - the gap between the dryer and the wall had an accumulation of debris.-Ensure the area is regularly cleaned3) Laundry room - there was an accumulation of old fridges, creating clutter.-Remove clutter from the facility as it can harbour pests

Broken vent panels have sharp edges that are accessible to kids. ****Vents in all other areas of the facility has been fixed***Action taken.-Operator was asked to place a barrier to limit children's access to the vent -Please repair the vent panels and keep classroom free from any physical hazard.
