      • 1) Dangling cords were observed in the clovers room.Operator took prompt action to secure them to the wall.2) In the sunshine room child (less than 3 years of age) was observed to be playing with small toys (possibly choking hazard).Small toys were removed from the childcare area during the inspection.3) Vinegar was observed to be stored in unlocked cabinets in one of childcare room.Violation was promptly addressed.

      • Highchairs and diapering pad straps were observed to be dirty with layer of grime.Concerns were discussed with the operator onsite.June 9th, 2026: Received pictures from operator confirming that the straps have been cleaned.

      • Following observations were made:1) Kids personal use beddings were observed to be placed togather.Concerns were discussed with operator and discussions were made to store these items in a way that they are not touching each other.

      • Toy bins in some classrooms were observed to have dust accumulation.- Clean and wash the toy bins when the toys are washed.

      • 1) Temperature of water at the handwashing sink in Acorns was measured at 57.8 and in Sunshines it was measured at 57.5- Reduce the water temperature to 54 degrees or below. Recommended 49 degrees C. 2) Push pins are in use throughout the facility. - Use double tape or hooks to secure art to the wall. 3) In willows room the child lock was loose such that it was posing a pinch hazard for clients.- Have the locks secured.

      • A water bottle in the acorns room had buildup on the straw. - The operator removed the bottle. Regularly check water bottles and replace as required.

      • 1) Gaps were observed between ceiling tiles throughout the facility.- Replace the ceiling tiles such that there are no gaps.2) In sunshine's room the caulking behind the handwashing sink was coming off. - Re-caulk the handwashing sink area.3) The island shelf between the Aspens BAS and BAS had the ply wood coming off exposing the wood underneath. - Repair the island.

      • 1) House structure in the reading nook in ferns was made of raw wood. - Sand, seal/paint the wooden structure. 2) The cash counter toy had raw wood exposed. - Reseal/ repaint the toy. 3) Wooden toys throughout the facility were worn off. -Re-sand/ replace the toys.

      • 1) Toy bins throughout the facility were observed to have dust accumulation.- Clean and wash the toy bins when the toys are washed. 2) The decorations in the sprouts room had dust accumulated.-Clean the decorations regularly.

      • In the BAS room a telephone with cord was present. - Remove the cord as it poses an entanglement hazard.

Worn and torn sleep mats were observed during the inspection.-Please repair or replace those mats to ensure properly cleaning/sanitizing of surfaces.3. Diapering table with worn surfaces exposing raw wood observed in facility's washroom.-Please repaint or replace and ensure smooth, non-absorbent and easy to clean surfaces.

      • Staff confirmed the sighting of a live mouse in the BAS (Before and After School) classroom. During the inspection, visible gaps were observed under the doors connecting the storage room and the BAS classroom, which may be contributing to pest entry.Required Actions:-Engage a licensed pest control professional to assess and eliminate all pest activity within the facility.-Seal all holes, crevices, and gaps—particularly under entrance and exit doors—to prevent further pest intrusion.-Submit biweekly pest control reports to the district inspector for continued oversight and compliance.

      • 1. Paint and other play items were stored in containers that were previously used for chemical storage. Do not reuse chemical containers for storage.2. A few rooms have wood stumps for chairs that have sharp pointy edges or large gaps where children could potentially get slivers. Smooth out or seal stumps to prevent injury. 3. A few outlets were not blocked throughout the facility. Ensure all outlets have covers.

      • Staff did not wash their hands or child hands after diapering and before picking up another child to diaper. Review diapering procedures with staff.

Ensure only diapering materials are stored on the diapering surface to prevent potential cross contamination.2. The safety belt for the change table located outside of the girls washroom is broken. Ensure this is repaired.

      • Wet cloth was left on top of the disinfectant bottle. Ensure cloths are submerged in disinfectant once wet or used one time to prevent potential growth of bacteria.

      • 1. There is a build up of debris and grim on the chairs throughout the facility. Ensure these are cleaned thoroughly. 2. The play mats in the Blossoms room were worn, had foam exposed, are in disrepair. Ensure these are removed.3. Several sleeping mats throughout the facility were in disrepair. Small holes can be patched with similar material. Larger ones should be replaced. 4. The yoga mats in Willows are in disrepair. Remove.5. One play table top in willow has raw wood exposed. Ensure this is resurfaced or removed.6. One shelf is in disrepair in the willow room. Repair or remove.7. The step stools in both bathrooms have raw wood exposed and are grimy. Replace.8. The play table in the Acorn room has raw wood exposed. Resurface or replace. 9. The large play blocks in the BAS program have a build up of grim. Ensure these are thoroughly cleaned.
