      • No disinfectant was observed in the facial treatment room to disinfect the facial massage probe. Ensure that a low level of disinfectant be used to disinfect the facial massage probe after each use.
    • 20. Is reusable equipment disinfected properly?
      • A bottle of Preempt CS 20 high level of disinfectant was equipped but it was expired. Ensure that a new bottle of high level of disinfectant be obtained. The manager reported that it has been ordered.No test strips for hydrogen peroxide were equipped in the facility. Ensure that the above test strips be obtained and equipped at all times.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Some foods such as instant noodles were observed in the reprocessing room. The manager reported that staff heats up lunches by using the microwave oven in the reprocessing room as well. Ensure that foods and other personal belongings be stored away from the reprocessing room. Since the microwave oven in the reprocessing room is deemed to be used for business purposes, staff must not use it to heat up lunches. Recommend that another microwave oven be equipped at the front kitchen area for staff use only. Ensure that the reprocessing room be not used for dining purposes and storage of foods and personal belongings at all times.

    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • The client records for tattoo services provided by the operator only had client's names and service dates. Ensure that all the required information be properly recorded according to the requirements of Personal Services Standards such as client's full name, contact information, service date and service description, etc. Client records must be kept accessible for review for a period of 2 years.
    • 33. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Non-Critical)?
      • Some oil was observed in a container without a lid. Ensure that oil used for massage be stored in a sealed container to prevent potential contamination. Recommend that the container be properly labelled.

      • The client records for tattoo services provided by the operator only had client's names and service dates. Ensure that all the required information be properly recorded according to the requirements of Personal Services Standards such as client's full name, contact information, service date and service description, etc. Client records must be kept accessible for review for a period of 2 years.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Used/soiled tools for pedicure services were observed being stored on the same shelve on a mobile cart. Ensure that clean tools and used tools be stored separately on the cart at all times to prevent contaminations. Clean supplies such as files and sponges were not individually packaged and were exposed to the air. Ensure that clean supplies be stored in enclosed containers at all times to prevent potential contamination.
    • 20. Is reusable equipment disinfected properly?
      • Tools were not immersed into the disinfectant and only the bottom parts of the tools came into contact with the disinfectant. Ensure that tools be completely immersed into the disinfectant when a high level of disinfectant is used as per required by both Personal Services Standards and manufacturer's instructions.In addition, no poured date of high level of disinfectant was recorded on either the sheet of concentration testing results or on the disinfecting container. Ensure that the poured date be clearly indicated either on the written record of the concentration testing sheet or be marked on the disinfecting container.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Hot chili sauce and salad dressings were observed in the reprocessing room. Note that no food prep, food storage and eating are allowed to occur in the reprocessing room.

      • No client records for tattoo services such as name, contact information and services date etc. were kept in place. Ensure that client records be kept properly and up to inspections.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Clean tools and used tools for pedicure were stored on the same shelving units on a cart. Ensure that clean tools and used tools are stored separately all the time to prevent contaminations.
    • 20. Is reusable equipment disinfected properly?
      • A Preempt CS 20 high level of disinfectant was used to disinfect all the reusable tools including semi critical tools. The operator intends to reuse the disinfectant. However, no pour out date or expiry date was labelled on the container. No concentration test records were kept in place either. If this high level of disinfectant is reused, the container must be clearly labelled with proper date and concentration of the disinfectant must be tested and recorded each time before it is used. Written records must be kept in place up to inspections.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • Some uncovered cushions were observed in the wet massage room. Ensure that proper covers be obtained to ease cleaning. In addition, combs used for head massage were observed dirty. They must be cleaned and disinfected after each use.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • Staff's lunch boxes were observed in the reprocessing room. Ensure that reprocessing rooms are not used for dining, food preparation and food storage at all times. In addition, designated clean area and dirty area must be completely separate at the reprocessing area.
