      • Following concerns were noted:1) Tall shelf in the infant room was not anchored to the wall.2) Cabinet in the infant room was observed to be unlocked.3) Medicinal rash cream was stored in the unlocked drawer under the diapering station, accessible to infants.4) Sliding door for the laundry room was getting off from its frame.5) Laundry sliding door in the childcare room was unlock.6) Storage cubby with hook in pre-school room was not anchored to the walls.Ensure to address the indicated violations in appropriate manner.-Anchor any cabinet/cubby with height more than 4 feet to the wall.-Either provide locks for all the accessible cabinets or remove any chemicals or medicine from the cabinets or shelves accessible to children.-Repair the laundry door, provide adequate locking mechanism to prevent children accessibility to laundry area.

      • The childcare room for children aged between 4 to 6 room had hot water kettle placed on the shelf accessible to children. Childcare staff took prompt action to remove the indicated kettle from the childcare room.
    • 06. Are proper protocols being followed in regards to client care?
      • Following concerns were noted regarding the diapering procedures:1) Disinfectant bottle near the diapering station in infant room was measured at 0 ppm.Use appropriate chlorine based sanitizer between 100 to 200ppm for sanitizing the diapering surface, after each use.2) Diapering mat was covered with fabric cover, that cannot be cleaned and sanitized between clients.Fabric covers must not be used to cover the diapering mats.3) Underlying diapering mat was ripped, exposing the non-smooth inner surface.Replace the diapering mat, so that its smooth, non-absorbent and easily cleanable.

      • The bags for the children were placed messily in the back area of the infant room. Some bags were open and on top of each other, touching other bedding.**Please arrange the storage of the bedding in the infant room. Place is neatly to ensure open bags are not on top of one another.**

      • Some of the white cubicles/ shelving units near the front door (that is being used to store children's personal belongings), has raw porous wood exposed along the edge trims.**Please install missing edge trims or apply a proper finish to the surface so it is made smooth, non-absorbent to moisture, and easily cleanable.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • There were no paper towels in the paper towel dispenser of the Toddler/Infant room. **Please replenish handwashing supplies and ensure hand sinks are adequately stocked with soap/paper towels at all times**
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • In the Infant/Toddler room, the back door has gaps exposed along the bottom corners to the exterior. Sunlight can be seen coming in through the gaps. ** Please repair or replace the weatherstripping of the back door and eliminate the gaps to the exterior. Ensure the door is properly weather/pest proof.
