    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Walls have damage throughout the facility with nicks, repair and repaint where needed.
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • The doors exiting into the playground in the preschool room are not properly pestproofed, specifically there are gaps under the door and allowing enough room for pests to enter. Ensure that all doors are properly pestproofed specifically that all gaps are all closed.

      • Please provide supporting documentation confirming that the mirrors in the classrooms on the wall are shatter proof; alternatively installed with 3m safety film the full length of the mirror.
    • 06. Are proper protocols being followed in regards to client care?
      • Replace the torn diaper change pad.
    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. Walls have damage throughout the facility with nicks, repair and repaint where needed.2. Replace the soiled ceiling tiles: As there are several soiled ceiling tiles throughout the facility, it appears that it may be a leak/ condensation issue; investigate the source of the leaks causing soiled ceiling tiles and provide supporting documentation to AHS.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Ensure that the paper towel dispensers in the washrooms are stocked with paper towels as all of the dispensers throughout the facility are empty.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. There is a fabric rug by the handwash sink in the preschool room and in the kitchen. It is absorbent to moisture and could move when walked on, posing a safety concern. Remove this rug entirely or replace it with a mat that is impermeable to moisture and does not move around.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Some of the children's cots/ sleep mats with bedding are touching and the storage bags are torn; replace all torn and worn bags.
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • The doors exiting into the playground in the preschool room are not properly pestproofed, specifically there are gaps under the door and allowing enough room for pests to enter. Ensure that all doors are properly pestproofed specifically that all gaps are all closed.

    • 06. Are proper protocols being followed in regards to client care?
      • 1. Currently the diaper change pad is covered with a blue tarp; as discussed use the change pad as purchased without a cover as the manufacturer's surface on the change pad is impermeable to moisture when intact. When it is not intact and the insulation of the change pad is exposed, replace the change pad.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • The staff washroom has a roll of paper towels on top of affixed paper towel dispenser; as discussed paper towels must be in a dispenser.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Sticky tiles in the children's washroom stall are coming off the floor; remove the lifting tile and replace the tile as discussed.2. The washroom garbage container requires hand contact to lift the lid. Replace the garbage container with one that does not require opening by hand3. There is a fabric rug by the handwash sink in the preschool room. It is absorbent to moisture and could move when walked on, posing a safety concern. Remove this rug entirely or replace it with a mat that is impermeable to moisture and does not move around.

    • 06. Are proper protocols being followed in regards to client care?
      • 1. Diaper change table belts are soiled.2. Do not cover diaper change mats with shower curtains. Acquire a new changing mat and repair the table.3. Improper hand wash practices for children and staff - no hand towels are being used.4. Hand dryer does not dry children's hands properly.
    • 09. Are properly operating hand washing sink(s) available in suitable location(s), and properly stocked with handwashing supplies?
      • Violations cited again on March 26. Fill the dispensers with paper towels.Hand wash basins in the washrooms did not have single use towels for children or staff.
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • 1. Multiple swelled and stained ceiling tiles are evident in the facility. Replace the ceiling tiles and investigate the leak and ensure that all damaged materials are replaced (insulation etc)2. Ensure that all shelves 4 feet or taller anchored to the wall.3. Repair the double doors to the exterior in the children's room.4. Repair the walls as necessary - refinish and repaint.5. Replace the broken diapering table held together with tape.6. Replace the soiled diapering mat that is covered with cloth cover and then wrapped in plastic. Ensure that it has a proper plastic cover in tact and belts that are not soiled.7. Window wells are damaged - refinish and repaint.8. Replace missing light covers.

      • A thorough cleaning of the entire facility is required including all hard-to-reach areas (behind and underneath mats and shelves)Ensure that all toys and surfaces are being cleaned and disinfected; provide a procedure for cleaning and disinfecting of mat tiles is conducted as they are soiled.
    • 27. Are proper measures being taken to ensure that vermin do not enter or exist in the facility (Non-Critical)?
      • Ensure that all doors are properly pestproofed.
    • 28. Are washrooms / bathing facilities properly constructed, maintained and supplied (Critical)?
      • The staff washroom had moisture build up while laundry was being done. The mirror was covered with condensation.Investigate the HVAC system which seems to be a concern throughout the facility. As condensation was evident on the windows. Outdoor temperature was 4C.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
      • Conduct a professional assessment of the building HVAC system; have the leak from the roof repaired causing issues with the ceiling tiles throughout the building.Have all of the vents and ducts cleaned in the building including the dryer.Painting ceiling tiles is not an acceptable option when they need to be replaced.

      • Plastic mat sleeves are being stored under the corners of the mats and accessible to children; which is a risk of asphyxiation.

      • Ensure that all doors are properly pestproofed.

    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1. A few staples in the Preschool room need to be taped to prevent staples from falling out and becoming a hazard to the children.2. The security gate is missing from the front hallway, and the preschool room door was open with no gate to block kids from leaving the class. Recommend replacing the gate at the front.
