- - There were two fly sticky traps hanging with significant number of dead flies in the hallway of the processing area.*Discard and replace the sticky fly trap.

- 1) The internal temperature of the bean sprouts in the walk-in cooler was measured at 13.5 Degrees Celsius.*Food was discarded and chemical was poured on top of it.**Adjust or repair the walk-in cooler to ensure it maintains a temperature of 4 degrees Celsius or lower.2) The thermometer inside the cooler was non-functional.*Have a functional and calibrated thermometer inside the cooler.

- 1) There was a mouse droppings observed on floor in the warehouse storage, by the water tank, by the hot water tank, upstairs storage, and along the entire back and side wall of bean sprout growing areas;2) A dead mouse was located on the floor of the upstairs storage area.*Remove and safely dispose all mouse feces and dead mice in the establishment and clean and disinfect all contaminated areas.

It's important to monitor this area so you can respond quickly.

- The bean sprout bins, equipment, utensils were not washed with soap/detergent and hot water, prior to sanitizing. -all food contact surfaces must be washed, rinsed and sanitized.

- 1) The handwash sink in the bean sprout processing area was blocked so it wasn’t easily accessible;2) There were no dispensers for soap or paper towel-the sinks can not be blocked, so ensure it is free at all times.-the sink must be operational, in good repair and has hot and cold running water.-install soap and paper towel dispensers.
