- **April 15, 2026 - Violation was still outstanding. Self monitoring pest control checklist was not completed. Traps were placed onsite. Observed pest control report from third party company was not dated and self monitoring pest control records were not being completed for the months that pest control does not conduct an inspection.

- The measured concentration of the chlorine sanitizer concentration from the sanitizer pail was excessive (1000ppm). Informed operator to ensure chlorine concentration is acceptable (100ppm).
- 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
- Hot rice was observed stored in beverage container coolers. Cooked green onion cakes were observed stored in soft plastic storage containers. Informed operator that food products can only be stored in food grade containers to prevent contamination from non food grade materials.

- Several food products in the upright coolers were not covered. Please ensure all food products are covered and protected from contamination.

- The handwashing station at the front counter was missing paper towels. Please ensure paper towels are available for all handwashing stations.

- A used large stainless steel pot was observed sitting on the floor of mop sink room. Informed operator to ensure all dirty equipment and utensils are placed into the dishwashing sink only.

- Spilled spices from containers were observed in several of the cabinets and drawers. Please ensure cabinets and drawers are properly cleaned.
