      • Contaminated equipment is being stored with single use ready to use and cleaned equipment. Ensure clear separation of clean, contaminated, and single use ready to use equipment.
    • 20. Is reusable equipment disinfected properly?
      • 1. There was physical residual at the bottom of the high-level disinfectant reservoirs. Ensure that items are fully cleaned prior to disinfection.Ensure that when high level disinfectant is changed, the containers are cleaned prior to being refilled. 2. Items were not being rinsed after submersion in high-level disinfectant. 3. High level disinfection testing logs are not being retained. Ensure to keep records of testing. 4. Two containers of high-level disinfectant did not pass their hydrogen peroxide test.

      • *One way flow for cleaning and disinfecting of equipment not observed. Area designated for cleaning and disinfection was noted to be needing cleaning and organizing. *Debris and dust were observed on equipment. Ensure equipment is adequately cleaned and disinfected.

      • Inadequate test strips. pH strips observed for CS 20 disinfectant.Ensure disinfectant is made using manufacturer instruction.

      • *Overall sanitation needs to be improved. Areas were discussed with the staff members onsite (i.e. dust, debris, garbage on the floor in one of the rooms, used equipment in the drawers, cabinets in the front area observed to be stained etc. )

      • *Evidence that singe use equipment was reused (i.e. nail buffers). Ensure all single use equipment is discarded after use. This was corrected onsite.

      • *One way flow for cleaning and disinfecting of equipment not observed. Area designated for cleaning and disinfection was noted to be needing cleaning and organizing. *Procedures for reuseable foot files for cleaning and disinfecting not in place. Ensure foot files are cleaned and disinfected adequately. *Debris and dust were observed on equipment. Ensure equipment is adequately cleaned and disinfected.
