      • 1. A bag of flour was observed stored directly on the floor in the dry storage room. The operator relocated it to a shelf. All food items must be stored at least 6 inches above the floor to prevent contamination.2. Raw meat was observed stored next to vegetables and above cooked foods in the upright freezer. Ready-to-eat foods and vegetables must be stored above raw meat and fish to prevent contamination.

      • The interior of the ventilation canopy has an accumulation of grease and dirt and must be thoroughly cleaned to prevent contamination.

      • Observed croutons being stored in cardboard box. Please put in a clean plastic container - better food storage and easily cleanable.

      • Additional cleaning for following items/area.- Filters for exhaust system require to be cleaned - observed accumulation of grease/debris.- Container under pizza boiler use to store pizza trays -> observed accumulation of food debris.

      • Dry storage room - observed a bag of onions and bag of flour stored directly on the floor. A bag of flour was left open (please seal it up while not in use).Additional attention on covering foods in the walk in cooler. Observed approx 4-5 large containers of cooked pasta uncovered.

      • Observed croutons being stored in cardboard box. Please put in a clean plastic container - better food storage and easily cleanable.

      • Dishwasher was not sanitizing. Checked a couple of times. Attempted to run/prime the line -> no sanitizer. Operator contacted tech and was going come onsite immediately.

      • No chlorine test papers. Operator indicated technician will be onsite - please ask them to supply you with chlorine test papers.

      • Dishwasher is not sanitizing (0 ppm chlorine was measured). Sanitizer hose appeared to be clogged and needs to be unclogged/replaced. Please ensure the dishwasher sanitizer is measuring at 100 ppm chlorine at final rinse.
    • 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
      • Please purchase chlorine test strips to test the dishwasher daily (100ppm required).

      • Soiled cardboards observed on multiple racks in the dry storage room. Please remove as these are not easy to clean and sanitize.

      • No sanitizer solution was prepared. A 100 ppm chlorine solution was made up at the time of inspection. Please ensure a sanitizer bucket is made up prior to doing any food prep for the day.

      • Dishwasher is not sanitizing (0 ppm chlorine was measured). Sanitizer hose appeared to be clogged and needs to be unclogged/replaced. Please ensure the dishwasher sanitizer is measuring at 100 ppm chlorine at final rinse.

      • Build-up of mold-like substance observed inside the ice machine. The lid of the ice machine was also in disrepair and covered with duct tape and raw wood (both not cleanable). Staff member indicated they are getting a new ice machine.

      • Soiled cardboards observed on multiple racks in the dry storage room. Please remove as these are not easy to clean and sanitize.
