April 25, 2024: A tape was used to cover the broken edges of the mirror as a temporary fix. The operator indicated that the mirror will be changed soon. Sept 16, 2024: The mirror in the toddler 1 room was still broken and covered with a tape. Please note that this is an on-going violation, and the broken mirror constitutes a hazard. Remove or replace the mirror.

Ensure an adequate fall protection is provided or remove the slide from the facility.

The staff prepared fresh 100ppm solution during inspection. Please note that 100ppm chlorine solution is required for normal operations and 1000ppm during an outbreak.
