- A mirror in toddler 1 room was broken at the lower left section, exposing sharp edges. Please replace the mirror or ensure this area is properly covered. April 25, 2024: A tape was used to cover the broken edges of the mirror as a temporary fix. The operator indicated that the mirror will be changed soon. Sept 16, 2024: The mirror in the toddler 1 room was still broken and covered with a tape. Please note that this is an on-going violation, and the broken mirror constitutes a hazard. Remove or replace the mirror.

It is recommended you provide information on the proposed material before purchase. April 25, 2024: The slides were still observed in the room but not in use. The operator indicated the required fall protective surfacing will be provided. Sept 18, 2024: A slide was still observed infant room. The staff stated that the slide is currently not in use. Ensure an adequate fall protection is provided or remove the slide from the facility.

The staff prepared fresh 100ppm solution during inspection. Please note that 100ppm chlorine solution is required for normal operations and 1000ppm during an outbreak.
