- Washrooms had some paper towel debris on the floor. Men's toilet had visible feces sprayed inside the toilet. PHI confirmed with operator that the check had just been completed, that this was observed, and that no action was taken. ACTION:Washroom checks need to accompany corrective actions to resolve poor sanitation in the washrooms. Washrooms should be maintained in clean and sanitary condition.
