      • Speakers with cords and personal items were stored on top of the diaper changing station in the infant room.The operator was instructed to remove the items away from the diaper changing station to prevent cross-contamination, choking hazards, and ensure the diaper changing station remains a sanitary surface.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • 1) Cabinets located in the infant room and the fridge located in the toddler room were not equipped with child safety locks.Please install child safety locks for the cabinets and the fridge.2) Various cubbies throughout the facility were observed to be unstable.Please inspect all cubbies and ensure they are secured and stable to prevent tipping3) Non-shatter proof mirrors were found in the preschool rooms.Please ensure the mirrors are equipped with a shatterproof film or remove them from the facility.

      • 1) Chlorine sanitizer solution was unavailable by the infant diaper changing station.The operator was instructed to make a chlorine spray bottle for the diaper changing station. Please ensure chlorine sanitizer solution measuring at 100ppm is available at all times for diaper changing.2) The surfaces under the diaper changing mat including the straps were soiled in dust and debris.The operator was instructed to clean and sanitize the surfaces including the straps.

      • 1) The turtle playmats located in the toddler room was peeling and in disrepair.Please remove the worn play mats.2) All rooms in the facility were observed to be dusty and soiled with debris.Please conduct a thorough deep cleaning of all rooms, including but not limited to floors, surfaces, fixtures, and furnishings.

      • The wooden slide located in the toddler room was measured at 24 inches. Indoor play equipment above 17.7 inches require and approved fall surfacing.The operator was instructed to remove the slide from the room.

      • 1) The handwash sink located in the washroom (left side) was continuously running.Please repair the handwash sink to ensure it can shut off.2) The cold-water handle on the handwash sink located in the washroom (left side) was observed to be cracked, creating sharp edges.Please repair or replace the handle to eliminate sharp edges and prevent potential injury.

      • Lighting in After School Care washroom is not well lit.- Please repair the light.

      • Update November 29, 2024:- Tape is present in both washrooms along the baseboard in the first washroom and behind the toilet in the furthest stall in the second washroom.- Walls need to be refinished in areas with drywall plaster. Ensure surface is smooth, non-absorbent, and easily cleanable.1. Wall in a washroom was tapped. - Repair the wall so it is easy to clean. ***Update April 12 24***Repeated violation - The damaged and taped wall in the washroom has not been fixed. Fix the wall immediately.2. Wall was damaged in a washroom where dispenser was removed. - Repair so the wall is smooth and easy to clean. ***Update April 12 24***Exposed dry wall was observed in the repaired area of the wall.Please paint the area where the repairs were done and ensure the wall surface is smooth, non-absorbent and easy to clean.

Foam floor tiles were stored on top of each other in the back toddler room creating a tripping hazard. 4.
