      • 1. Staff personal items were in physical contact with other staff personal items. This was corrected during the inspection.2. Toys were being stored in the furnace room. Please remove all toys from furnace room and ensure the door remains locked at all times.3. Linens were noted to be stored in the staff washroom/laundry. Please do not use washroom for storage. Please remove all items.

      • Indoor play area was renovated without submitting plans for approval. Please ensure all renovations and constructions plans are properly submitted for approval prior to starting construction.
    • 19. Are indoor play areas and equipment sufficient and being properly maintained to ensure client safety (Critical)?
      • Play equipment including climbing structures measuring 40 inches, as stated by the operator, was seen in the indoor play area without proper fall surfacing. All elevated play equipment was removed from the play area and stored in the baby room as the baby room is currently not in use. Elevated play equipment above 20 inches in height is not allowed unless adequate testing has been completed for the indoor surfacing.Please ensure there is adequate supervision when the walking blocks play equipment is in use to ensure it is played with appropriately and to prevent climbing on the structure.

      • Locks were not observed on all mechanical doors. Please intall locks in all mechanical doors.

      • 1. A hole was noted in the washroom wall of the OSC room and mechanical room door of the toddler room. Please seal all these holes. 2. A transition piece was missing by the OSC room door. Please install the missing transition piece.

      • 1. Paint was noted to be peeling from following areas:- diapering station located in the baby room. - Cabinet underneath the hand washing station's location in baby and toddler room. Please paint all indicated areas and ensure they are maintained in a smooth, moisture resistant and easy to clean state. 2. Ceiling tiles were noted to be damaged int eh washroom located int eh toddler room and OSC room. Please replace all damaged ceiling tiles.

      • Accumulation of dust was noted on all ceiling vents as well as mechanical ventilation system located in washrooms. Please clean all vents.

      • Towels, toys and extra children clothing were noted to be stored int eh washroom. Please do not store facility related items in the washroom.

      • Clients' personal items were in physical contact with other clients' items. Please ensure that all clients personal items are physically separated.Please ensure that staff items are properly labeled as well.

      • It was noted that parents provided food items are being reheated in the baby room. A hand washing sink is not observed in this room. Please either install a hand washing sink or reheat food items in the commercial food establishment.

      • Outbreaks and illnesses are not being reported to AHS. The childcare director indicated that they were not aware of illness reporting policy or that these needed to be reported to AHS. Discussed illness reporting and advised the operator to read the policy.

      • 1. Proper diapering procedures were not being followed. Please train staff members.2. Clients' hands were not being washed after diapering. Please wash clients' hands. 3. Proper hand washing was not being performed by staff members after diapering. Please educate staff on importance of hand washing. 4. Diapering area was not being disinfected properly. Please educate staff on importance of disinfection of diapering area.

      • Proper toy washing procedures were not being performed. The staff indicated that they wash toys in a being with soap, water and bleach. Proper toy washing procedures were discussed with the operator during the inspection. Please wash toys in the OSC room sinks. Please wash toys with soap and water, rinse with clean water and then disinfect using bleach solution.

      • Containers of cereal and clean dishes were noted in the OSC room within client's access. Please be advised that all food items and clean dishes must be stored in a locked cabinet at all times.

      • 1. Wall stapler was noted in the OSC room. 2. Chemicals were noted in the washroom located in the OSC room. 3. Construction materials were noted in the common area. 4. Garbage bins and recycles bins were noted within client's access. Please remove all hazards from all rooms.
    • 06. Are proper protocols being followed in regards to client care?
      • Hand washing was not being performed before and after lunch/snacks. Please ensure that clients hands are being washed properly.

      • 1. A designated toy washing sink was not observed in the facility. The operator was advised to either to use sinks located in the OSC room or in the kithcen area. 2. Paint was observed to be peeling from multiple surfaces including inside storage cabinets, play structures, walls., etc. Please refinish all surfaces and ensure these are maintained in a smooth, easy to clean and moisture resistant state at all times. 3. Mold was noted in the cabinet located underneath the three-compartment sink in the OSC room. Please clean and remove/replace all surfaces that contain mold. 4. Cracks were noted in the plexigalss that is being used as a guardrail in the Kinder room. A packing tape was being used to seal these cracks. Please be advise that taping cracks is not a appropriate solution. Please replace the plexigalss.

      • 1. Soiled laundry containers were stored within client's access. Please be advised that soiled laundry containers must not be stored within client's access. 2. Soiled laundry was observed to be overflowing from the bin in the baby room. Please increase the frequency of laundry and do not overflow bins.

      • 1. General cleaning and organizing is required throughout the facility. Please clean and organize the facility thoroughly and ensure it is maintained in a clean and sanitary state at all times. 2. The couch and a chair located in the kinder area were observed to be in disrepair. Please repair or replace the couch and a chair. 3. Chairs noted in the baby room were in insanitary state. Please replace these chairs and ensure that all materials are maintained in a clean and sanitary state.

      • 1. Toys were not being properly washed/disinfected.Proper toy washing procedure was discussed with the operator. Please wash toys with soap and water, rinse with clean water and then disinfect using bleach solution in the OSC room. Please create a schedule for each room and implement it. 2. Mats that are being used underneath the indoor slide are in insanitary state. Please replace these mats.

      • Sleeping mats were observed to be in disrepair and are no longer smooth, and easy to clean. Please either repair or replace these sleeping mats.

      • 1. Clients' personal items were in physical contact with other clients' personal items. Please be advised that all clients items must be physically separated. 2. Mattresses were being stored in the crib in the baby room. Please designate an area to store mattresses. Please do not use cribs for storage purposes.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • One of the washrooms in the OSC room was not in an operational state. Please repair the washroom.
    • 30. Are workers washing hands when appropriate and/or in a proper manner?
      • Staff members were not washing hands prior to serving lunch. Staff members were observed to be using gloves. Importance of hand washing was discussed with the operator during the inspection. Glove usage does not replace hand washing. Hands must be washed using soap and water and dried using a single use disposable paper towel.

      • An odor was noted in the baby room and the laundry room. Please investigate the source of the odor and re-medaite it.

      • A mouth toy bin was not observed in any room at the time of inspection. Please be advised that a designated mouth toy bin must be available in all rooms.

      • 1. It was noted that the facility was using OSC kitchenette for food prep and dishwashing purposes. Clean and dirty dishes, a toaster oven, potato peelers etc. were noted in this kitchenette. Walls and door were not observed around this kitchenette and this area was noted to be within clients' access. Please be advised either to remove all food utensils, equipment and prep items from this kitchenette and use this area for serving purposes only or ensure that this area is fully enclosed with walls and a door and ensure clients do to have access to this area. Please submit a copy of renovation plans to this area.

      • Sleeping mats were observed to be in disrepair and are no longer smooth, and easy to clean. Please either repair or replace these sleeping mats.
