- Containers of cereal and clean dishes were noted in the OSC room within client's access. Please be advised that all food items and clean dishes must be stored in a locked cabinet at all times.

- It was noted that the facility was not following proper dishwashing procedures. Please be advised to follow following procedures:- wash with clean water in the first sink, - rinse in the second sink,- prepare a bleach solution at 100 ppm in the third sink and leave clean dishes fully submerge in the bleach solution for two minutes. Please ensure to use a bleach test strip to test the concentration. - Air dry the dishes.

Paint was observed to be peeling from cabinet located underneath the sink. Please repair the cabinet. 3. The countertop by the sink was damaged. Please repair the countertop.

- It was noted that the facility was using OSC kitchenette for food prep and dishwashing purposes. Clean and dirty dishes, a toaster oven, potato peelers etc. were noted in this kitchenette. Walls and door were not observed around this kitchenette and this area was noted to be within clients' access.Please be advised either to remove all food utensils, equipment and prep items from this kitchenette and use this area for serving purposes only or ensure that this area is fully enclosed with walls and a door and ensure clients do to have access to this area. Please submit a copy of renovation plans to this area.

- 1. Carpet was noted in the staff room which is being used for food storage area. Please ensure that all surfaces are made of smooth, easy to clean and moisture resistant state. 2. Employee washroom door was observed to be open. Please install a self-closing mechanism to this door and keep it closed at all times.
