- 02. Has the operator notified the RHA of the existence of a personal service business, including contact and location information, as well as all personal services currently offered or new personal services that will be offered for facilities and/or special events?
- The owner of the personal service business did not submit a notification about the business to the Alberta Health Services. Submit the notification to AHS.

- No written procedures for skin cleansing and antiseptic use prior to piercing services were available. No post-service care instructions were provided after piercing services. Please ensure that the instructions are provided verbally and in writing.

- The operator showed lack of knowledge of related to provision of proper piercing services: used unapproved product for piercing gun disinfection, used unapproved product for skin preparation prior to piercing services, lack of understanding of sterility documentation retention requirement and provision of post-service care instructions (verbal and written).

- A hand sink located in the waxing room was not supplied with soap.

- Unacceptable antiseptic product was used for skin preparation prior to piercing: no DIN or NPN was available.

- Sterility assurance documentation for sterile equipment used for piercing was not retained with the clients' records. Ensure that sterility assurance documentation for all critical equipment used during services is retained with clients' forms.

- Barbicide solution was used against manufacturer's instructions: it was not changed daily. Ensure that Barbicide solution is changed daily.

- Some cosmetic products (YonKa, Danbys line of 5 products, 24 Carat Gold Bleach and D-Tan) did not have appropriate labels. Ensure that all cosmetic products have appropriate labels or discontinue use.

- There was no means provided for hand drying in the washroom. Please provide paper towels or single-use towels in an appropriate dispenser, or provide a hand dryer.
