      • *Recurrent Violation* The outdoor playground uses wood/bark chips for fall surfacing. While there appears to be an acceptable quantity of bark chips (dug to a depth of 8-10" and still could go deeper), there were several areas where the bark chips had compacted; it does not appear that routine raking/tossing of these wood chips is being done.REQUIREMENT: Bark/wood chips need to be periodically raked/tossed to ensure the chips are not compacting; do this for the entire playground, with special focus on areas where children are elevated and at a greater risk of falling (e.g. monkey bars, swings).
