- Mouse droppings were observed on the floor under shelving unit by the stove as well as in the far back kitchen area in the hard-to-reach areas of the floor. Please safely clean up the droppings and sanitize the affected areas. As well please email the undersigned PHI copies of pest control records.

- 09. Are chemicals stored and handled in a safe manner?
- The Quat sanitizer bottle was unlabeled. Please ensure all chemical bottles are labeled such that the contents can be identified and safely used. Staff labeled that bottle at the time of the inspection.
- 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
- The facilities Quat test strips had expired in 2014 and 2015. Please obtain new Quat test strips to be able to accurately test and verify the Quat sanitizer solutions being used in the facility.

- Mouse droppings were observed under the pots and pans storage shelf by the six-burner stove as well asl along where the wall meets the floor in the back kitchen area. Please safely clean the droppings up and sanitize the affected areas. Please contact the professional pest control company to service the facility.
