      • Observation: Screen on screen door not properly affixed to screen door. Screen door does not completely close and leaves greater than 1/4 inch gap at the bottom.
      • Corrective Action(s): Operator to ensure screen on screen door is properly affixed. Screen door to be repaired or replaced to ensure no gaps when door is closed to prevent pest entry. Screen door is not to be used until it is repaired or replaced. Back door must be closed at all times.

      • Observation: Spiders and spider webs noted in corners above standing cooler, on shelving that stores cleaned and sanitized pots and pans aross standing cooler, and in the chair storage area outside of the kitchen. Old mouse droppings noted under the handwashing sink. Debris and dust noted on floors and in hard-to-reach areas.
      • Corrective Action(s): Operator to ensure spiders and webs are removed. All stored pots and pans must be cleaned and sanitized again. Old mouse droppings are to be removed using a mild bleach solution at 100ppm. Floors are to be cleaned and sanitized as per the Sanitation Plan. Noted areas must be maintained in a sanitary condition.

      • Corrective Action(s): Throughly clean / vacuum spider webs and maintain those areas in good sanitary condition.

      • Observation: 1. Plastic wall cover over hand wash station is beginning to peel off. Paint over the wall cover is starting to degrade.
      • 2. Ceiling area above the hand wash station patched with cardboard.
      • Corrective Action(s): 1. Repair or affix the wall cover so that the material around the hand wash sink is smooth, free of cracks, and easily cleanable.
      • 2. Cardboard should be removed and properly re-patched and re-painted.
