      • While walking around the baby room a child was observed to be fast also on top of a pile of cushions and stuffies. The staff member indicated the child was having a power nap. PHI had the staff immediately lift up the child which she held in her arms as he woke. PHI instructed if the child is to need to continue their sleep it must only be in the crib or cot

      • The bleach solution in Explorer 1 room was not properly labelled with its contents.Hand soap in the Navigator room was not labelled with its contents.

      • Three screws were protruding from the train table in the Navigator 1 Room.
    • 22. Is the plumbing system adequate and being properly maintained (Non-Critical)?
      • No hot water was available at the hand sink in the Navigator room.

      • The shelving unit in Pioneer 1 was not secured from tipping and was over 4 feet in height. Please secure.
