- A dirty cleaning/sanitizing rag was observed placed on the counter next to a flour bin, another rag was found next to the fryer, and a third rag was placed inside the sink.Ensure that all cleaning rags are stored in a container of sanitizing solution when not in use, or placed with soiled linens to be laundered after it's completed use. Cleaning/sanitizing rags must not be left on food contact surfaces, near food preparation areas, or inside sinks.

- The hand sink adjacent to the front counter was not operational at the time of inspection.The issue was discussed with the manager on site. The sensor was cleaned, and the sink was restored to proper working condition during the inspection.Ensure that all hand sinks are fully functional and accessible at all times. Regular checks and maintenance should be conducted to prevent recurrence.

- The hand soap dispenser in the staff washroom was found to be empty at the time of inspection.Ensure that all handwashing stations are fully stocked with an adequate supply of hand soap at all times. Regular monitoring and replenishment should be included in routine checks to support proper hand hygiene practices.

- 1. A dead mouse was found in the basement furnace room.2. Mouse dropping were observed in the furnace room.Ensure that The dead mouse and droppings are safely removed and disposed of. The affected area should be thoroughly cleaned and sanitized.Although comprehensive inspections of the facility are conducted by a licensed pest management company, ensure documentation of this pest incident is recorded.

- 1. A water leak was observed from the faucet in the mopping sink area. 2. A wall tile is missing from the mop cleaning sink area, which may allow water to penetrate and cause damage within the wall.3. Light fixtures in the food preparation area and storage room do not have adequate protective coverings or are missing end caps.Ensure that,1. Faucet is repaired to stop the leak and keep the area clean and dry.2. Repair the wall, so this area is cleanable and impervious to moisture/water intrusion.3. Appropriate light protective covers and end caps are properly fitted, secured, and maintained to prevent contamination.

- A water leak was observed coming from the drain line of over-head AC unit in the walk-in freezer, resulting in dripping water that had frozen on the floor and on food boxes stored underneath.The source of the leak should be identified and repaired. Ensure that accumulated ice on the floor is thoroughly removed. Store any food items away from this area to prevent potential contamination.

- The floor area next to the 3 compartment sink and walk-in cooler was found to be visibly dirty, and had an accumulation of water Identify the source of water and repair to prevent further water accumulation. The affected area should be thoroughly cleaned and sanitized. Put this area in the routine cleaning schedule to ensure that it remains clean and dry.

- The automatic liquid hand soap dispenser in the men's staff washroom, located in the basement was not working. An orange light would flash when triggered for soap, but no soap would dispense.Repair or refill the soap dispenser in the men's staff washroom.
- 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
- The shatterproof light covers in the walk-in cooler were observed to not be fully protective of the light bars. This would allow glass to escape containment in the event of a breakage. Have maintenance repair or replace the light covers or install a shatter resistant bult (ie: LED).

- A light over the sink area in the men's customer washroom, was not working making it quite dark in the washroom.Repair the light so that it is working, to provide adequate light levels in the men's washroom.

- The paper towel dispenser for the front service area hand wash sink was not dispensing paper towels.The batteries for the paper towel dispenser were replaced, and then the paper towels dispensed.
