    • 06. Are proper protocols being followed in regards to client care?
      • Diapering procedures with gloves were not followed properly. Please ensure gloves are removed and changed after removing the children's diaper.Operators were educated on diapering procedures during the inspection
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. Various sinks throughout the facility had grouting that was dirty and in disrepair (including but not limited to toddler room 1 and preschool room 1).In areas where the grout is in disrepair, please replace the grout, and clean and sanitize the area around the sink. 2. the diapering table in toddler room 1 was visibly dirty along the edge.Please clean and sanitize the diapering area so it remains in a sanitary condition3. the diapering mat in between toddler room 2 and 3 was damaged making it difficult to clean.Please replace the diapering mat to ensure it is smooth, non-absorbent, and easy to clean4. The table in the school age room was damaged on the trim making it difficult to clean. Please repair or replace the table so it is smooth, easy to clean, and non-absorbent

      • Toys being washed in the classrooms were being washed, rinsed, and sanitized in the same sink compartment. Soap can impact the effectiveness of bleach and washing and rinsing in the sanitizing area can result in contamination.To ensure toys are washed effectively, either wash all toys in the kitchen's commercial dishwasher, or acquire large bins in each room to use for the wash and rinse step and designate the sink for sanitizing.

    • 06. Are proper protocols being followed in regards to client care?
      • Toddler 2/3: Improper diapering procedures were observed:Staff did not sanitize the diapering surface immediately after use. Staff only sanitized the diapering mat but did not clean areas where soiled wipes were placed.Please ensure all diapering areas, nearby surfaces, and any contaminated items are thoroughly sanitized after each use.Toddler 2/3: Toys were dried on a diapering surface instead of designated tables. Please ensure toys are dried on the appropriate tables.Please review the diapering procedures with the staff.

      • 1. Infant 2, Toddler 1, Preschool 3: The garbage bin’s foot pedals are malfunctioning. Please repair or replace as needed.2. Toddler 1: The recycle bin was grimy. Please clean and sanitize thoroughly.3.Preschool 1, Toddler 1: The inner edges of the toy basket are dirty. Please clean and sanitize thoroughly.4. Toddler 4: The edges by the sink are in disrepair. Please repair them.

      • Improper toy cleaning procedures were observed:- The diapering hand sink was used to clean toys.- Staff indicated that a dedicated sink is not used for toy washing.Please ensure the following: - diapering sink is used only for diapering-related activities and that toys are washed in a separate sink that is not used for handwashing or using a dishwasher.- review toy cleaning procedures with the staff

      • Infant 4: Dirty toys were stored near the toilet. Please ensure dirty toys are stored away from the toilet until they are cleaned.

      • Toddler 4: The diapering mat is torn. Please replace it.

      • Toddler 1: One of the bleach bottles was missing a label. Please ensure it is properly labeled.

    • 08. Is this facility properly constructed (Non-Critical)?
      • 1. There were several water-stained ceiling tiles in the School Age Room. Some water-stained tiles were observed in the hallway as well. **Please determine the source of water allowing for the staining and properly repair (if it has not been repaired already). Then, remove water-stained ceiling material and replace with new materials. **Continue to monitor for additional staining. 2. In the Toddler Room, the teal-colored wall near the sink had patches of missing paint, exposing a surface difficult to clean/disinfect.**Please apply a proper finish and ensure the wall is smooth, non-absorbent to moisture, and easy to clean.
    • 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
      • 1. The inner seal around the washer in the laundry room was mouldy. ** Please wash mould off with soap and water. If the mould cannot be washed off, remove the seal, and replace with new materials. 2. There was an accumulation of lint beside / behind the washer/dryer.**Please ensure the lint is removed and the area cleaned/ disinfected. Please check to ensure the dryer vent is operating properly to remove lint.3. In the school age room, the faucet in the washroom is loose at the base and stained.** Please clean / disinfect the area and then properly secure the faucet to the sink. Ensure a water tight joint is formed. 4. The washroom ceiling vent fans throughout the facility had an accumulation of dust/debris and does not appear to be cleaned regularly. ** Please ensure the vent fan is cleaned so it can operate as intended. Ensure the cleaning of these areas are put on the cleaning schedule.
    • 25. Are there adequate storage areas sufficient for the operation of the facility?
      • 1. In the infant room, there was a blue mouthing ring toy that was parent provided, but unlabeled. **Please label all personal items. 2. There was a spoon stored on the shelf in the fridge. ** Please ensure the spoon is stored is a safe and sanitary manner. The fridge shelf surface may not necessarily be clean.
    • 29. Are washrooms / bathing facilities properly constructed, maintained and supplied (Non-Critical)?
      • In the toddler room, one of the parent provided toilet seats (that was on the toilet) were badly soiled and had an accumulation of urine/feces in the cracks/crevices around the seat. It did not appear to be cleaned/disinfected regularly or thoroughly. ** Please ensure the item is properly cleaned/disinfected.
