      • 1. The shelving used to store children's personal items in the toddler room, pre-k room, and out of school care room was in disrepair and the wood finish was peeling off. Please repair or replace the shelving so the finish is smooth, durable, and easy to clean.2. The cupboard near the hand sink in the baby room was in disrepair and the wood finish was peeling off.Please repair or replace the shelving so the finish is smooth, durable, and easy to clean.3.       • The kitchen door was unable to lock due to missing components.Please repair/install a lock on the kitchen door to prevent access into the kitchen.

      • - The wires connected to the music player was dangling in the Pre-K room. ** Please ensure the wires are secured to the wall and unable to be accessed by children.** This was corrected during the inspection. Staff taped up the wires behind the shelves to make them inaccessible. - CORRECTED** -
    • 06. Are proper protocols being followed in regards to client care?
      • - After a diaper change in the Baby Room, staff put the dirty diaper in a bag, sealed it, and put it on the floor. The staff also took off the gloves used and put it on the floor. ** Please relocate the garbage bin and store it near the diapering station so staff can put the gloves and the diaper in the bin, instead of on the floor. **This was corrected during the inspection** -

      • - 1. One of the tall shelves in the baby room had an edge trim where raw wood was exposed. **Please install a proper edge trim or apply a proper finish so the surface is made smooth, non-absorbent to moisture, and easy to clean. 2. Some of the shelves holding the toy bins throughout the facility were worn, with some sections of the wood that was missing sections. ** Please repair or replace the shelves and ensure the surfaces are smooth, non-absorbent to moisture, and easy to clean. -

    • 31. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Critical)?
      • The pressure from the infant handwashing sink has increased, to the point where residual dirty spray/splashing from hand hygiene is contaminating other surfaces including the infant high tables where food is consumed.REQUIREMENT: Investigate and fix the spray from the infant handwashing sink so that the spray does not cross contaminate critical surface like children's food contact areas.
