    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The 70% isopropyl alcohol used for antisepsis was observed to not have an NPN or DIN.Please ensure that products used for the antisepsis of skin are applied after a cleansing step, used as per manufacturer's instructions, and have either a DIN or NPN.
    • 20. Is reusable equipment disinfected properly?
      • Barbicide was mentioned by the operator to be used to reprocess lash tweezers at the time of inspection. Expired CS 20 was also observed at the time of inspection.Please ensure that semi-critical tools such as lash tweezers are cleaned and disinfected using, at minimum, a high-level disinfectant.

      • In a conversation with the operator, it was mentioned that not all sterility documentation is retained with client records. A discussion was held with operator of the requirements and importance of retaining sterility documentation with client records.Please ensure that sterility documentation of any pre-sterilized, disposable product that is intended to puncture the skin is retained with client records to help ensure proper recordkeeping in the case of infections or recalls. Sterility documentation can include but is not limited to lot numbers, expiry dates, manufacturer's instructions/information.

      • Ear piercing cartridge packaging was observed to be missing sterility documentation.Please ensure that sterility documentation is available for all equipment that is purchased as sterile and is intended to puncture the skin to help confirm that the item is sterile.

      • The operator had mentioned that semi-critical equipment was being re-processed in the building washrooms. The operator was informed and asked to reprocess the equipment in an area that did not contain a toilet. A kitchenette in the upstairs portion of the building will be used as a temporary location until a plumbed-in sink is installed in the service area. Please ensure that an appropriate and designated reprocessing area is used to clean and disinfect tools.

      • Written procedures were not available at the time of inspection. Please ensure that written procedures detailing each step for each service is retained onsite for reference to help ensure safe practices.

      • The operator mentioned that they immerse their semi-critical tools in CS 20 for about 10 minutes. Contact times were reviewed with the operator. A disinfectant log was not available. Please ensure that semi-critical tools are immersed for 20 minutes to ensure sufficient contact time. Please also ensure that the log template that is sent with the report is regularly completed.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • A needle cartridge was observed to have no lot number, expiry date or indication of sterility at the time of inspection. When asked, the operator mentioned that they were not using the product. Operator was asked to discard the product.Please ensure that any equipment that is intended to be sterile has the documentation associated with the item to ensure its sterility and safe use.

      • Manufacturer's instructions for laser and other facial machine equipment were not available at the time of inspection.Please provide and have the manufacturer's instructions for use available for reference.

      • The mobile sink was observed to only be dispensing hot water at the time of inspection.Please ensure that the mobile sink is able to dispense both hot and cold water.
