- Bacteriostatic saline was going to be used for reconstitution of Botox product. Bacteriostatic saline cannot be used to reconstitute Botox as per the manufacturer's instructions because it contains a preservative. After the inspection, the operators submitted photos of preservative-free, sterile saline that will be used instead.

- Written procedures were not prepared, but the operator filled them out shortly after the inspection and submitted them to the PHI.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Facility had not been retaining sterility information/lot numbers for microneedling tips. This was discussed during the inspection and lot numbers will be recorded on each client form moving forward.
