      • Observation: 1) Leak observed at 3 compartment sink faucet.
      • 2) Hot water at staff washroom handsink stops running after a couple seconds when turned on.
      • 3) Paper towel dispenser broken in staff washroom.
      • Corrective Action(s): 1) Tighten/repair faucet.
      • 2) Repair handsink to have hot and cold running water available.
      • 3) Repair/replace paper towel dispenser.

      • Observation: -Main pizza prep cooler found to be at 23C. Facility was aware that it was broken and was already taking steps to minimize impact to operations
      • Corrective Action(s): -Very small amount of toppings were being brought out at a time to ensure rapid use. Ice packs or frozen water in inserts need to be used in addition to ensuring small amounts of foods are brought out to provide some cooling capacity. A replacement cooler has been ordered and is due to be installed on Thursday of this week. Re-inspection will occur to ensure replacement cooler is installed.

      • Observation: Observed live mouse running through kitchen during inspection.
      • Rodent droppings observed throughout facility per code 306.
      • Corrective Action(s): CLOSURE ORDER ISSUED
      • As discussed in 306, all droppings must be removed. Pest control must be contacted for a visit immediately

      • Observation: Giant hole in ceiling tiles above walk in cooler. Seal has also come off the wall underneath the 3 compartment sink.
      • Corrective Action(s): Close off the hole/gap and re-seal the part underneath the 3 compartment sink.

      • Observation: Rodent droppings observed throughout facility, along the floors underneath the handwash station, by the walk in cooler, underneath the pop rack.

      • 2) sanitize affected areas with bleach/water
      • 3) Contact pest control for a visit immediately

      • Observation: Rodent droppings observed along wall underneath the handwash station and beside the walk in cooler
      • Corrective Action(s): Remove all droppings and sanitize affected areas. Pest control visits are currently weekly but there appears to still be ongoing issues. If the outcome does not improve, facility may be closed to ensure that business has adequate time to properly address/correct ongoing rodent issues.

      • Observation: As discussed with staff and owner, there are numerous gaps in the ceiling that could be providing entry points for the rodents to enter into the kitchen area.
      • Corrective Action(s): These gaps and open areas must be closed/sealed in order to prevent rodents from gaining access to your kitchen. As discussed, it is the business operator's responsibility to ensure that these hazards are minimized and corrected
