    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • There were no written procedures for the cleaning of skin before providing services and for accidental skin cuts.The PHI reviewed the written procedures with the operator and asked to keep the records of the same.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Paper towels and single-use towels were kept at the corners of the sink (not protected from contamination).
    • 16. Is reusable equipment handled and stored properly (Non-Critical)?
      • There was no container to store used single use microfiber hand towels. Provide a labeled container in the washroom by the hand sink to store used hand towels.
    • 20. Is reusable equipment disinfected properly?
      • Test strips for high-level disinfectant (PREempt CS20) were not available. Obtain high-level disinfectant specific test strips and test the high-level disinfectant daily and maintain daily test log on the AHS template provided during inspection onsite.

      • A bottle containing a product (unidentified) was noted in the service area without label to identify its contents.Operator was advised to adequately label all the containers/bottles to identify their contents correctly.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The service chair (for clients) was worn, rendering it not easily cleanable.The operator was advised to cover the chair with an easily cleanable and non-porous material.
    • 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
      • -Reprocessing sink was located in the room with a toilet. Operator was advised to remove the toilet and cap the drain properly to prevent the emission of sewer gases in the reprocessing area. Operator was further advised to stop offering services until a reprocessing sink is available. Remove the toilet as discussed onsite and cap the drain. Send pictures after removing the toilet to the undersigned for review.
