      • Bingo markers, not suitable for children were observed in one of the child care rooms.They were removed during the inspection.
    • 04. Are clients at this facility being protected from hazards (Non-Critical)?
      • A dollhouse located in the toddler room was observed to be unstable and posed a potential tripping and safety hazard. The concern was discussed with the operator during the inspection.Later the same day, the operator provided photographs confirming that the dollhouse had been removed and discarded, as it could not be securely anchored to the wall.The violation was corrected and addressed.

      • During the inspection, the safety belt on the diapering mat in the toddler room was observed to be torn, raising concerns regarding its integrity, effectiveness, and ability to be properly cleaned and disinfected.The concern was discussed with the operator. The damaged area was temporarily repaired during the inspection to prevent further deterioration.Following the inspection, the operator provided documentation confirming that a replacement safety belt had been ordered.The operator was advised to ensure that the replacement safety belt is installed promptly and maintained in good repair to support safe diapering practices and effective cleaning and disinfection.

      • Evidence of a significant number of ants and flies were observed along the wall behind the sensory table located in the Tiger Room.Please address the pest issue and send a pest control report to the undersigned PHI. Ensure the area is closed off to children until the issue has been addressed.

      • A cloth seat located in the Monkey Room was observed to be stained and covered with marker.Please launder the seat covers.
