      • 1. A light cover in the OSC room was observed sagging down.Adjust the light cover to ensure it is properly installed and prevent potential injury from shattered glass.2. An electrical outlet was observed without an electrical outlet cover in the OSC room. Place a cover over the outlet to mitigate electrical hazard risks.3. The cord of the wall mounted TV was freely dangling in the OSC room. Secure the cord to the wall to prevent any entanglement hazard risks.

      • The paper towel dispenser in the washroom and infant room were empty. Refill the paper towel dispenser and ensure all hand washing sinks are fully stocked with soap and paper towels at all times for proper hand hygiene.

      • The neck of the disinfectant bottle for the diapering area was in disrepair. Replace the spray nozzle or bottle.

      • A green fabric foam mattress was observed underneath the white plastic diapering mattress. Remove or cover in plastic the green fabric mattress. Ensure surfaces of the diapering station have a smooth and easy to clean surface for effective disinfection to prevent the spread of communicable diseases and infections.

      • The temperature throughout the facility was below 20C. Operator states facility temperature is controlled throughout the building.Raise and maintain all room temperatures to a minimum of 20C.

      • Blankets in the infant/toddler room were stored together without any separation. Operator moved blankets into designated cubbies. Store blankets with child's individual items to protect them from contamination.

      • Fridge thermometers were unavailable in the preschool and OSC rooms.Ensure thermometers are available in refrigerators to accurately maintain temperatures 4C or below.
