      • Observation (CORRECTED DURING INSPECTION): Surface sanitizers ("No-rinse sanitizer") available for use. However, as per the manufacturer's inspection, to be used for food contact surfaces, the surfaces must be rinsed with potable water after use. Staff made 200ppm chlorine solution and filled both spray bottles at this time. Staff was advised to use the solution to sanitize all food prep surfaces at this time.
      • Corrective Action(s): Prepare 200ppm chlorine solution by adding 1/2 tsp of bleach per 1L of water and refill the bottle every 2 weeks.

      • Observation (CORRECTED DURING INSPECTION): Upon arrival of EHO, staff were marinating chicken in a tupperware while using a wastebin as a prep surface.
      • Corrective Action(s): Do NOT continue this practice. Properly sanitized prep surfaces must be used for all food prep.

      • Observation (CORRECTED DURING INSPECTION): Observed 2 inserts of frozen meat thawing at room temperature at the cook line.
      • Corrective Action(s): Do not thaw at room temperature. All frozen foods must be thawed in the cooler or under cold running water in the prep sink. Staff moved the 2 inserts back into the cooler.

      • Observation: Heavy rodent acitivity is present in the facility. Droppings are present along the entire perimeter of the kitchen, leading into the storage closet at the cashier station, and into the customer area behind stored chest freezers.
      • The most recent pest control report from March 27, 2024 indicated 5 live catches, and 100% bait eaten in all exterior stations. Next pest control visit is scheduled for April 16, 2024.
      • Corrective Action(s): CLOSURE ORDER ISSUED.
      • Premises is to close IMMEDIATELY and remain closed until the following corrective actions have been completed:
      • > Pull all equipment at least 2ft from the walls to fully access the affected areas.
      • > Ensure all items that are stored on the ground are lifted.
      • > Remove ALL rodent droppings.
      • > Clean and sanitize affected areas with a strong bleach solution.
      • > Fill and seal all entry points such as cracks and crevices.
      • Do not re-open until a follow-up inspection has been conducted and written approval is granted.

      • Observation: - Mouse droppings present along edge of floor in closet behind service area. Droppings appeared to be limited to this area.

Continue to monitor for any further activity ensuring any additional droppings present are cleaned up promptly. Continue to maintain regular pest control service. Have pest control technician place trap in this area.

      • - REPEAT VIOLATION: Mouse droppings observed along the edges of wall inside kithcen.

      • - This is ongoing issue. Operator has professional pest control company visiting every month. However sanitation is not being maintained. Operator do not have physical copies of the report available. Send the most recent report to the undersigned by 9am of Feb 21, 2023. CLEAN and SANITIZE all areas affected with rodent droppings.

      • - No sanitizer was available in kitchen.

      • - Do not dilute the premixed sanitizer. Spray bottles were filled with the sanitizer measured at 200ppm QUAT.
      • - Bleach was available and 200ppm bleach sanitizer was made in red bucket for kitchen use. Ensure to prepare sanitizer at the start of the day.

      • Observation (CORRECTED DURING INSPECTION):
      • - No paper towel available at the kitchen handsink.
      • Corrective Action(s):
      • - Ensure handsinks are fully equipped with paper towel and hand soap at all times. Paper towel was repleshed at the time of inspection.

      • Observation:
      • - Front door open.
      • Corrective Action(s):
      • - Premises is located at the second floor of shared building and can be entered by the front lower level door or back second floor level (from parking lot). Both doors were open at the time of inspection and the store entrance door was open as well. Ensure to close the two doors (entrance to the building) OR the entrance door to the store at all times to prevent pest entry. Consider installing door screen.

      • - Rodent droppings observed along the corner/wall junctions, behind & under equipment and hard-to-reach areas.

      • - Grease & dirt debris build up observed behind & under equipment and by the mop bucket.
      • Corrective Action(s):
      • - Clean the areas noted above.
      • - Spray 1000 ppm bleach sanitizer on the areas affected with rodent droppings. This can be made by mixing 1 part bleach with 50 parts water.
      • - Remove newspapers. Line the shelving units with a material that can be easily cleaned.

      • Observation: Health Approval NOT granted as high temperature dishwasher did not achieve 71C. The dishwasher was run 5x and was not consistantly increasing in temperature. Dishwasher temperature on the 5th run was 65.9C. Have dishwasher repaired/replaced so it is capable of sanitizing (achieve 71C rinse / 82C at gauge) then call for reinspection.
